不同麻醉方式对膝关节韧带损伤患者术后关节功能恢复的影响

摘　　要：目的:探究全身麻醉与硬脊膜外腔阻滞麻醉对膝关节韧带损伤患者术后关节功能恢复的影响。方法:选取2023年5月—2024年1月聊城市中医医院收治的90例膝关节韧带损伤患者为研究对象,随机将其分为对照组和研究组,各45例。对照组采用全身麻醉,研究组硬脊膜外腔阻滞麻醉,比较两组关节活动度、疼痛程度、膝关节功能及膝关节功能恢复情况。结果:术后第1、3、5、7天及术后第2、4、6周,研究组关节活动度均高于对照组,差异均有统计学意义(P<0.05)。术后6、24、48 h及1周,研究组视觉模拟评分法评分均高于对照组,差异均有统计学意义(P<0.05)。术后第6、12周,研究组Lysholm膝关节评分均高于对照组,差异均有统计学意义(P<0.05)。研究组膝关节功能恢复优良率为93.33%,高于对照组的86.67%,差异有统计学意义(P<0.05)。结论:相较于全身麻醉,硬脊膜外腔阻滞麻醉能够更好地促进膝关节韧带损伤患者术后膝关节功能的恢复,降低疼痛程度,提高关节活动度。Objective:To explore the effects of general anesthesia and epidural space block anesthesia on the recovery of joint function in patients with knee ligament injury after surgery.Methods:A total of 90 patients with knee ligament injury admitted to Liaocheng Hospital of Traditional Chinese Medicine from May 2023 to January 2024 were selected as the study objects,and randomly divided into a control group and a study group,with 45 cases in each group.General anesthesia was used in the control group,and epidural space block anesthesia was used in the study group.Joint motion,pain degree,knee function and knee function recovery were compared between the two groups.Results:At the 1st,3rd,5th,7th day and 2nd,4th and 6th week after surgery,the joint motion of the study group were higher than those of the control group,the differences were statistically significant(P<0.05).At 6,24,48 h and 1 week after operation,the visual analog scales score of the study group were higher than those of the control group,with statistical significance(P<0.05).At the 6th and 12th week after surgery,Lysholm knee joint scores in the study group were higher than those in the control group,with statistical significance(P<0.05).The excellent and good rate of knee function recovery in the study group was 93.33%,which was higher than 86.67%in the control group,the difference was statistically significant(P<0.05).Conclusion:Compared with general anesthesia,epidural block anesthesia can better promote the recovery of knee function,reduce the degree of pain and improve the range of joint motion in patients with knee ligament injury after surgery.
