桃红四物汤联合五苓散加减对老年股骨粗隆间骨折患者PFNA内固定术后的康复效果分析

摘　　要：目的:分析桃红四物汤联合五苓散加减对老年股骨粗隆间骨折(IFOF)患者股骨近端防旋髓内钉(PFNA)内固定术后的康复效果。方法:选取2020年4月—2024年4月东莞市长安医院收治的82例老年IFOF患者作为研究对象,按照随机数字表法分为对照组和试验组,每组41例。两组均进行PFNA内固定术,对照组在实施PFNA内固定术后,采用利伐沙班片联合塞来昔布胶囊治疗,试验组在对照组基础上采用桃红四物汤联合五苓散加减治疗。比较两组疼痛程度、髋关节功能和凝血功能指标。结果:治疗后,试验组疼痛视觉模拟评分低于对照组,Harris髋关节功能评分量表评分高于对照组,差异均有统计学意义(P<0.05);治疗后,试验组纤维蛋白原和D-二聚体水平均低于对照组,凝血酶原时间和活化部分凝血活酶时间均短于对照组,差异均有统计学意义(P<0.05)。结论:桃红四物汤联合五苓散加减应用于老年IFON患者PFNA内固定术后康复中,有助于减轻术后疼痛,改善凝血功能指标和髋关节功能。Objective:To analyze the rehabilitation effect of Taohong Siwu Decoction combined with Wuling Powder added or decreased in the treatment of elderly patients with intertrochanteric fracture of femur(IFOF)after internal fixation of proximal femoral nail anti-rotation(PFNA)internal fixation.Methods:A total of 82 elderly patients with IFOF admitted to Chang’an Hospital of Dongguan City from April 2020 to April 2024 were selected as the research objects and divided into a control group and an experimental group according to random number table method,with 41 cases in each group.Both groups were treated with PFNA internal fixation.The control group was treated with Rivaroxaban Tablets combined with Cecoxib Capsule after surgery,and the experimental group was treated with Taohong Siwu Decoction combined with Wuling Powder on the basis of the control group.Pain degree,hip function score and coagulation function index were compared between the two groups.Results:After treatment,the visual analogue score of experimental group was lower than that of control group,and the score of Harris Hip Function Scale was higher than that of the control group,with statistical significance(P<0.05).After treatment,the levels of fibrinogen and D-dimer in experimental groups were lower than those in control group,and the prothrombin time and activated partial thrombin time in experimental groups were shorter than those in control group,with statistical significances(P<0.05).Conclusion:Taohong Siwu Decoction combined with Wuling Powder added or decreased can reduce postoperative pain in elderly patients with intertrochanteric fracture of femur after PFNA internal fixation,improve coagulation index and hip function.
