摘　　要：目的:讨论钩针透壁缝合法固定直疝假性疝囊在腹腔镜经腹腔腹膜前疝修补术(TAPP)中的应用效果。方法:选取2022年1月—2023年9月在佛山市第二人民医院采取TAPP手术治疗的腹股沟直疝患者78例,采用计算机随机数字法分为试验组(n=39)与对比组(n=39)。两组均进行TAPP,试验组术中采取钩针透壁合法固定直疝假性疝囊,对比组术中采取常规缝合法固定直疝假性疝囊。比较两组术后疼痛程度、住院时间及术后血清肿发生率。结果:试验组与对比组术后数字评定量表评分对比,无统计学差异(P>0.05);试验组住院时间短于对比组,差异有统计学意义(P<0.001)。试验组血清肿发生率低于对比组,差异有统计学意义(P=0.006)。结论:TAPP术中采取钩针透壁缝合法固定假性疝囊,能够降低术后血清肿发生率,缩短住院时间。Objective:To discuss the application effect of crochet through wall suture in fixing false hernia sac of direct hernia in laparoscopic transperitoneal hernia repair(TAPP).Methods:A total of seventy-eight patients with direct inguinal hernia treated by TAPP surgery in Foshan Second People's Hospital from January 2022 to September 2023 were selected and divided into experimental group(n=39)and control group(n=39)by computer random number method.TAPP was performed in both groups.The experimental group was fixed by crocheting through the wall,while the control group was fixed by conventional suture.The degree of postoperative pain,length of hospital stay and incidence of postoperative seroma were compared between the two groups.Results:There was no significant difference between experimental group and control group in postoperative Numerical Rating Scale score(P>0.05);the duration of hospital stay in the experimental group was shorter than that in the control group,and the difference was statistically significant(P<0.001).The incidence of seroma in experimental group was lower than that in control group,and the difference was statistically significant(P=0.006).Conclusion:Fixing pseudohernia sac with crochet through wall suture during TAPP can reduce the incidence of postoperative seroma and shorten the length of hospital stay.
