储继军基于“内伤脾胃,百病由生”理论辨治慢性盆腔痛经验

摘　　要：慢性盆腔痛(CPP)是妇科难治性疾病之一,中医学将其归属于“妇人腹痛”范畴。CPP病因多样,病机复杂,病程缠绵,严重影响女性生活质量及生殖健康。目前,西医临床以消炎镇痛等对症治疗为主,短期疗效显著,但症状易反复发作,长期效果欠佳。中医学理论认为,冲任虚衰,胞脉失养,不荣则痛及冲任瘀滞,胞脉不畅,不通则痛为CPP发病直接原因。虚、瘀在CPP的发生发展及病理改变中具有重要作用。储继军主任医师遵从中医学治病原则,正确辨证论治,合理遣方用药,充分发挥中医优势,根据“内伤脾胃,百病由生”理论,结合长三角地区妇人中医体质特点,认为CPP病机以脾虚为本,湿、热、寒邪为标,瘀血阻滞为发病因素。治疗上总以健脾贯穿始终,临证加减,同时指导患者调整饮食、生活习惯以矫正脾虚体质,临床疗效良好。Chronic pelvic pain(CPP)is one of the refractory diseases of gynecology,which belongs to the category of“women′s abdominal pain”in traditional Chinese medicine.The causes of CPP are varied,the pathogenesis is complex,and the course of the disease is protracted,which seriously affects the quality of life and reproductive health of women.At present,western medicine mainly focuses on symptomatic treatment such as anti-inflammatory and analgesia,which has significant short-term effect,but the symptoms are prone to repeated attacks and the long-term effect is poor.According to the theory of traditional Chinese medicine,pain caused by loss of nutrition due to deficiency and weakness of Chongchannel and Ren-channel and malnutrition of uterus vessels,and pain caused by loss of smoothness due to static Chongchannel and Ren-channel and obstructed uterus vessels are the direct causes of CPP.Deficiency and stasis play an important role in the occurrence,development and pathological changes of CPP.Chief physician Chu Jijun follows the principles of TCM treatment,treats the disease by correct syndrome differentiation,reasonably prescribes and uses medicines,which give full play to the advantages of TCM.According to the theory of“Internal Impairment of the Spleen and Stomach Caucusing Various Diseases”and the characteristics of TCM constitution of women in Yangtze River Delta region,Chu Jijun believes that the pathogenesis of CPP was based on spleen deficiency,with pathogenic factors of dampness,heat and cold as the manifestations and blood stasis as the pathogenic factors.The treatment is to strengthen the spleen throughout and modify the drugs according to the symptoms.At the same time,the patients are instructed to adjust their diet and living habits to correct spleen deficiency constitution.The clinical efficacy is good.
