T2WI联合DWI及MRI增强扫描指导直肠癌术前T分期的价值

摘　　要：目的分析T2加权成像(T2WI)联合弥散加权成像(MRI)增强扫描指导直肠癌术前T分期的价值。方法回顾性分析2020年7月至2024年3月收治的72例直肠癌患者的临床资料,患者均进行T2WI、DWI、MRI增强扫描检查,以手术病理结果作为“金标准”,分析单独T2WI、T2WI联合DWI及MRI增强扫描对直肠癌术前T分期的诊断价值差异。结果72例直肠癌患者术后病理检查T分期结果为:T1期者9例、T2期者15例,T3期者21例,T4期者27例;T2WI对T1、T2、T3、T4的诊断准确率分别为55.56%、60.00%、76.19%、96.30%,T2WI对直肠癌患者T分期的总诊断准确率为77.78%(56/72);T2WI联合DWI及MRI增强扫描对T1、T2、T3、T4的诊断准确率分别为77.78%、86.67%、85.71%、100.00%,T2WI联合DWI及MRI增强扫描对直肠癌患者T分期的总诊断准确率为90.28%(65/72);T2WI对直肠癌患者T分期的总诊断准确率低于T2WI联合DWI及MRI增强扫描对直肠癌患者T分期的总诊断准确率(χ^(2)=4.191,P=0.041)。结论T2WI联合DWI及MRI增强扫描联合对直肠癌术前T分期的诊断准确率较高。Objective To analyze the value of T2-weighted imaging(T2WI)combined with diffusion weighted imaging(DWI)and magnetic resonance imaging(MRI)enhanced scan on guiding preoperative T staging of rectal cancer.Methods The clinical data of 72 patients with rectal cancer in the hospital were retrospectively analyzed from July 2020 to March 2024.All patients received T2WI,DWI and MRI enhanced scan.The surgical pathology result was used as the gold standard to analyze the diagnostic value of T2WI alone,T2WI combined with DWI and MRI enhanced scan on preoperative T staging of rectal cancer.Results T staging results of postoperative pathological examination of 72 patients with rectal cancer revealed T1 stage in 9 cases,T2 stage in 15 cases,T3 stage in 21 cases and T4 stage in 27 cases.The diagnostic accuracy rates of T2WI for T1,T2,T3 and T4 stages were 55.56%,60.00%,76.19%and 96.30%respectively,and the total diagnostic accuracy rate of T2WI for T staging of rectal cancer was 77.78%(56/72).The diagnostic accuracy rates of T2WI combined with DWI and MRI enhanced scan for T1,T2,T3 and T4 stages were 77.78%,86.67%,85.71%and 100.00%,respectively,and the total diagnostic accuracy rate for T staging of rectal cancer was 90.28%(65/72).The total diagnostic accuracy rate of T2WI for T staging in patients with rectal cancer was lower than that of T2WI combined with DWI and MRI enhanced scan(χ^(2)=4.191,P=0.041).Conclusion T2WI combined with DWI and MRI enhanced scan has high diagnostic accuracy rate for preoperative T staging of rectal cancer.
