国医大师周学文脾胃病临证撷英

摘　　要：国医大师周学文一生躬身临床,深谙医理,精于辨证,融会贯通,师古而不泥古,衷中又笃西。其在消化领域提出了多项创新理论及治法:消化性溃疡“毒热”病因理论及清热解毒、消痈生肌治法;溃疡性结肠炎清热除湿,化浊运脾治法;胆汁反流性胃炎“胆火逆胃”学说及疏肝清胆,和胃通降治法;胃食管反流病“气”“热”“郁”的核心病理因素及降逆平冲,清热解郁治法;功能性消化不良调气畅中,疏肝理脾治法;腹泻型肠易激综合征健脾化湿,调和五脏治法等。本文列举周师治疗脾胃病的临床经验,管窥其学术理念,以飨同道。National traditional Chinese medicine Master ZHOU Xuewen,dedicated his life to clinical practice,had a deep understanding of medical principles,and was highly skilled in syndrome differentiation and treatment.He integrated ancient wisdom with contemporary insights,respected the past without being confined by its limitations,and was devoted to both traditional Chinese and Western medicine.In the field of digestion,he proposed several innovative theories and treatment methods:the"toxic heat"etiology theory and the"treating ulcers as boils"academic thought for peptic ulcers;the method of clearing heat and removing dampness,transforming turbidity,and regulating the spleen for ulcerative colitis;the"gallbladder fire reversing to the stomach"theory and the method of soothing the liver,clearing the gallbladder,and harmonizing the stomach and descending treatment for bile reflux gastritis;the core pathological factors of"Qi","heat",and"depression"for gastroesophageal reflux disease,and the method of descending reversal,calming the stomach,clearing heat,and resolving depression;regulating Qi and promoting the flow of the middle burner,soothing the liver,and regulating the spleen for functional dyspepsia;and the method of regulating the spleen,transforming dampness,and harmonizing the five viscera for diarrhea-predominant irritable bowel syndrome,etc.This article lists several clinical experiences of Master ZHOU Xuewen in treating spleen and stomach diseases,providing a glimpse into his unique academic concepts and benefiting fellow practitioners.
