摘 要:目的分析尿清蛋白(urinary albumin,UAlb)/肌酐(creatinine,Cr)、血清胱抑素C(serum cystatin C,CysC)在诊断早期糖尿病肾病(diabetic nephropathy,DN)中的价值。方法选取2022年1月—2023年12月盐城市第一人民医院收治的86例糖尿病(diabetes mellitus,DM)患者为研究对象,依据尿蛋白排泄率将其分成早期DN组(≥300 mg/24 h,26例)与单纯DM组(<300 mg/24 h,60例)。比较两组CysC、UAlb/Cr水平,并绘制受试者操作特征(receiver operating characteristic,ROC)曲线,计算曲线下面积(area under the curve,AUC),评定CysC、UAlb/Cr单独及联合检测对早期DN的诊断效能。结果早期DN组的CysC、UAlb/Cr水平高于单纯DM组,差异有统计学意义(P均<0.05)。ROC曲线结果显示,CysC与UAlb/Cr联合检测的AUC高于单纯UAlb/Cr、CysC检测,且其灵敏度、特异度更高。结论针对早期DN患者,UAlb/Cr与CysC联合检测的诊断效能优于单一指标检测。Objective To analyze the value of urinary albumin(UAlb)/creatinine(Cr)and serum cystatin C(CysC)in the diagnosis of early diabetic nephropathy(DN).Methods A total of 86 patients with diabetes mellitus(DM)admitted to Yancheng First People's Hospital from January 2022 to December 2023 were selected as the research objects.Ac⁃cording to the urinary protein excretion rate,they were divided into early DN group(≥300 mg/24 h,26 cases)and simple DM group(<300 mg/24 h,60 cases).The levels of CysC and UAlb/Cr were compared between the two groups,and the receiver operating characteristic(ROC)curve was drawn.The area under the curve(AUC)was calculated to evaluate the diagnostic efficacy of CysC and UAlb/Cr alone and in combination for early DN.Results The levels of CysC and UAlb/Cr in the early DN group were higher than those in the simple DM group,and the differences were statistically significant(both P<0.05).ROC curve results showed that the AUC of combined detection of CysC and UAlb/Cr was higher than that of simple UAlb/Cr and CysC detection,and the sensitivity and specificity were higher.Conclusion For patients with early DN,the diagnostic efficacy of combined detection of UAlb/Cr and CysC is better than that of single index detection.
