达格列净联合胰岛素治疗老年糖尿病肾病的临床效果及对肾功能及炎症因子水平的影响

摘　　要：目的分析老年糖尿病肾病(diabetic nephropathy,DN)患者联合使用达格列净、胰岛素的治疗效果。方法选择2021年10月—2023年10月邹城市第二人民医院收治的80例DN患者为研究对象,以治疗方法的不同分为A组(40例,行胰岛素治疗)和B组(40例,行达格列净+胰岛素治疗),对比两组肾功能指标、炎症因子水平、血糖水平。结果治疗后,B组尿素氮、血肌酐、β2-微球蛋白、甲状旁腺激素、白细胞介素-6、C反应蛋白、肿瘤坏死因子、白细胞介素-1β、空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于A组,差异有统计学意义(P均<0.05)。结论老年DN患者联合使用达格列净、胰岛素治疗,可显著改善肾功能、炎症因子及血糖水平。Objective To analyze the therapeutic effect of combined use of dapagliflozin and insulin in elderly pa⁃tients with diabetic nephropathy(DN).Methods A total of 80 patients with DN admitted to Zoucheng Second People's Hospital from October 2021 to October 2023 were selected as the research objects.According to different treatment methods,they were divided into group A(40 cases,treated with insulin)and group B(40 cases,treated with dapa⁃gliflozin+insulin).The renal function indexes,inflammatory factor levels and blood glucose levels of the two groups were compared.Results After treatment,the levels of blood urea nitrogen,serum creatinine,β2-microglobulin,para⁃thyroid hormone,interleukin-6,C-reactive protein,tumor necrosis factor-α,interleukin-1β,fasting plasma glucose,2-hour postprandial plasma glucose and glycated hemoglobin A1c in group B were lower than those in group A,and the differences were statistically significant(all P<0.05).Conclusion The combination of dapagliflozin and insulin in elderly patients with DN can significantly improve renal function,inflammatory factors and blood glucose levels.
