摘 要:目的分析高频超声诊断2型糖尿病颈动脉病变与其视网膜血管病变的关系。方法选取2023年6月—2024年6月莆田市第一医院收治的96例2型糖尿病患者为研究对象,所有患者均行检眼镜、眼底荧光造影检查,根据检查结果进行分组,32例纳入糖尿病无视网膜病变组(NDR组),28例纳入非增殖型糖尿病视网膜病变组(NPDR组),36例纳入增殖型糖尿病视网膜病变组(PDR组)。同时纳入30名健康体检者作为对照组。对所有患者均行常规超声检查,并以速度向量成像技术(velocity vector imaging,VVI)脱机处理二维颈动脉图像。对比各组颈总动脉内中膜厚度(intima-media thickness,IMT)、血流参数、最大应变(maximum tangential strain,Smax)、收缩期最大运动速度(systolic maximum velocity,Vs)、最大应变率(maximum tangential strain rate,SRmax),分析IMT、Smax、Vs、SRmax与视网膜病变级别之间的相关性。结果NPDR组与PDR组患者的IMT水平均明显高于对照组,PDR组IMT水平高于NDR组患者,差异有统计学意义(P均<0.05)。PDR组Smax、Vs、SRmax均低于NPDR组、NDR组、对照组,差异有统计学意义(P均<0.05)。2型糖尿病患者IMT水平与其视网膜病变严重程度呈正相关,Smax、Vs、SRmax与视网膜病变严重程度呈负相关(P均<0.001)。结论高频超声技术可用于2型糖尿病患者的颈动脉病变的诊断,并且其颈动脉病变情况与视网膜微血管病变程度显著相关。Objective To analyze the relationship between high-frequency ultrasound diagnosis of carotid artery dis⁃ease and retinal vascular disease in type 2 diabetes mellitus.Methods A total of 96 patients with type 2 diabetes mel⁃litus admitted to the First Hospital of Putian City from June 2023 to June 2024 were selected as the study objects.All patients underwent ophthalmoscopy and fundus fluorescein angiography.According to the results of the examination,32 patients were included in the non-diabetc retinopathy group(NDR group),28 patients were included in the nonproliferative diabetic retinopathy group(NPDR group),and 36 patients were included in the proliferative diabetic reti⁃nopathy group(PDR group).At the same time,30 healthy subjects were included as the control group.All patients un⁃derwent conventional ultrasound examination,and two-dimensional carotid artery images were processed offline by ve⁃locity vector imaging(VVI).The intima-media thickness(IMT),blood flow parameters,maximum tangential strain(Smax),systolic maximum velocity(Vs)and maximum tangential strain rate(SRmax)of common carotid artery in each group were compared.The correlation between IMT,Smax,Vs,SRmax and the grade of retinopathy was analyzed.Results The IMT levels of the NPDR group and the PDR group were significantly higher than those of the control group,the IMT level of the PDR group was higher than that of the NDR group,and the differences were statistically significant(all P<0.05).The Smax,Vs and SRmax of the PDR group were lower than those of the NPDR group,the NDR group and the control group,and the differences were statistically significant(all P<0.05).IMT level was posi⁃tively correlated with the severity of retinopathy in patients with type 2 diabetes mellitus,while Smax,Vs and SRmax were negatively correlated with the severity of retinopathy(all P<0.001).Conclusion High-frequency ultrasound tech⁃nology can be used for the diagnosis of carotid artery disease in patients with type 2 diabetes mellitus,and its carotid arter
