基于能谱CT测量的胰腺细胞外容积分数与高血压相关性的研究

摘　　要：目的:探讨基于能谱CT碘基图获得的胰腺细胞外容积分数(fECV)与高血压相关指标之间的相关性,并评估其在高血压患者胰腺慢性损伤中的应用价值。方法:回顾性收集行上腹部能谱CT增强扫描的患者233例,根据血压情况将其分为血压正常组122例和高血压组111例。高血压组中1、2、3级高血压分别47、29、35例。结果:高血压组胰腺fECV的中位数(35.88%)显著高于血压正常组(32.92%)差异有统计学意义(P<0.001),且随高血压等级的升高,胰腺fECV也升高。胰腺fECV与收缩压、舒张压、脉压、高血压等级均呈正相关(r=0.54,0.41,0.41,0.55;均P<0.001)。结论:基于能谱CT所测得的胰腺fECV与高血压相关指标呈正相关,其中高血压等级是胰腺fECV升高的独立危险因素;fECV在研究高血压对胰腺的慢性损伤方面具有较高的临床价值。Objective:To investigate the correlation between pancreatic extracellular volume fraction(fECV)measured by spectral CT iodine maps and hypertension-related indicators,and to evaluate its application value in chronic pancreatic injury among hypertensive patients.Methods:A retrospective study was conducted,including 233 patients who underwent abdominal spectral CT enhanced scans.233 patients were divided into the normal group(122 cases)and the hypertension group(111 cases).Among the hypertension group,47 cases were grade 1 hypertension,29 cases were grade 2,and 35 cases were grade 3.Results:The median value of pancreatic fECV in the hypertension group was 35.88%,significantly higher than that in normal group of 32.92%(P<0.001).Moreover,with the increasing grade of hypertension,pancreatic fECV also increased.Pancreatic fECV showed positive correlations with systolic pressure,diastolic pressure,pulse pressure and hypertension grade(r=0.54,0.41,0.41 and 0.55,all P<0.05).Conclusions:Pancreatic fECV measured by spectral CT has a positive correlation with hypertension-related indicators,with hypertension grade being an independent risk factor for increased pancreatic fECV.In hypertensive patients,fECV measured by spectral CT has high clinical value in studying chronic pancreatic injury related to hypertension.
