摘 要:射血分数保留的心力衰竭(HFpEF)约占全部心力衰竭的50%,是一组高度异质性的以左心室舒张功能受损为主的疾病,常伴有高血压、糖尿病、肥胖和心房颤动等合并症,且传统心力衰竭治疗方式不能完全改善其临床结局。近年来研究发现,脂肪组织分泌的脂联素和瘦素可能通过多种机制参与HFpEF的病理生理机制。本文总结了脂联素和瘦素与HFpEF的关系,并进一步讨论了脂联素悖论和瘦素抵抗的潜在机制,以期为HFpEF的治疗提供新的靶点和基础理论。Heart failure with preserved ejection fraction(HFpEF),which accounts for approximately 50%of all heart failure,is a highly heterogeneous group of disorders with predominantly impaired left ventricular diastolic function,often associated with comorbidities such as hypertension,diabetes mellitus,obesity,and atrial fibrillation,and conventional heart failure treatments do not fully improve clinical outcomes in HFpEF.Recent studies have revealed that adipose tissue-secreted lipocalin and leptin may be involved in the pathophysiology of HFpEF through multiple mechanisms.This article summarises the relationship between lipocalin and leptin and HFpEF,and further discusses the potential mechanisms of lipocalin paradox and leptin resistance,in order to provie new targets and basic theories for the treatment of HFpEF.
