腰麻联合硬膜外腔麻醉在高龄髋部骨折患者手术中的应用效果分析

摘　　要：目的:探究腰麻联合硬膜外腔麻醉在高龄髋部骨折患者手术中的应用价值。方法:选取2022年2月—2024年2月江油市人民医院收治的高龄髋部骨折患者72例为研究对象,随机分为参照组和观察组,各36例。参照组给予硬膜外腔麻醉,观察组给予腰麻联合硬膜外腔麻醉。比较两组麻醉效果、循环功能、麻醉指标、不良反应。结果:观察组麻醉总有效率高于参照组(P=0.024);麻醉后10 min,两组心率、收缩压、舒张压水平低于麻醉前,观察组高于参照组(P<0.05);观察组麻醉起效时间、运动和感觉阻滞恢复时间、术后清醒时间短于参照组,麻醉药总用量少于参照组(P<0.001);观察组不良反应发生率低于参照组(P=0.024)。结论:腰麻联合硬膜外腔麻醉在高龄髋部骨折患者手术中的麻醉效果更理想,可减少麻醉药总用量,确保患者麻醉安全性,并维持机体循环功能正常运转,促进患者术后快速清醒,术后尽早恢复感觉、运动阻滞和精神意识,麻醉安全性更高。Objective:To investigate the application value of lumbar anesthesia combined with epidural anesthesia in the surgery of elderly hip fracture patients.Methods:Seventy-two cases of elderly hip fracture patients admitted to Jiangyou People's Hospital from February 2022 to February 2024 were selected for the study,and were randomly divided into the reference group and the observation group,each with thirty-six cases.The reference group was given epidural anesthesia,and the observation group was given lumbar anesthesia combined with epidural anesthesia.Compare the anesthesia effect,circulatory function,anesthesia indexes and adverse reactions of the two groups.Results:The total effective rate of anesthesia in the observation group was higher than that of the reference group(P=0.024);10 min after anesthesia,the heart rate,systolic blood pressure and diastolic blood pressure levels of the two groups were lower than those before anesthesia,and the observation group was higher than that of the reference group(P<0.05);the onset time of anesthesia,the recovery time of motor and sensory blockade,and the time of postoperative wakefulness of the observation group was shorter than that of the reference group,and the total dosage of anesthesia drugs was less than that of the reference group(P<0.001);the incidence of adverse reactions in the observation group was lower than that in the reference group(P=0.024).Conclusion:Lumbar anesthesia combined with epidural anesthesia is more ideal in the surgery of elderly hip fracture patients,which can reduce the total dosage of anesthetics,ensure the safety of anesthesia for the patients and maintain the normal function of circulatory function of the body,and promote the patients to wake up quickly after the operation,and to recover the sensory and motor block and mental consciousness as early as possible after the operation,and the safety of anesthesia is higher.
