基于R语言数据挖掘探讨中药内服治疗膝关节置换术后疼痛的用药规律

摘　　要：目的:基于数据挖掘技术分析中药口服处方治疗膝关节置换术后疼痛的用药规律。方法:通过检索中国知网、万方数据、维普数据库和SinoMed等数据库筛选中药治疗膝关节置换术后疼痛的相关文献,建立“膝关节置换术后疼痛处方用药数据库”,采用Excel整理并统计所有方药的信息,包括单味中药的使用频次、性味、归经,利用R语言软件对其进行频数、关联规则及层次聚类分析。结果:共纳入80首处方,涉及药物145味,用药总频次为984次,前5味高频中药为当归、川芎、红花、牛膝、桃仁,性味以甘、苦、温居多,归经以归肝经、脾经为主;关联规则分析提示桃仁、红花、当归、川芎为核心药物;通过聚类分析获得14类药物组合。结论:中医治疗膝关节置换术后疼痛以活血通经、化瘀止痛为用药思路,可根据辨证论治结果在核心组方的基础上加减其他中药,为今后临床治疗提供借鉴。Objective:The medication rules of oral Chinese medicine on postoperative pain in knee arthroplasty was analyzed based on data mining technology.Methods:Relevant literature on treating postoperative pain in knee arthroplasty with oral Chinese medicine was screened by databases such as CNKI,Wanfang Data,VIP database,and SinoMed to establish a database of prescription medication for postoperative pain in knee arthroplasty.Excel was used to arrange and statistically analyze information on all prescriptions,including the used frequency,medical property and flavor,and meridian of single Chinese medicine.The frequency,association rules,and hierarchical clustering analyses were conducted by R language software.Results:A total of 80 prescriptions were included,involving 145 Chinese medicines with a total frequency of 984 times.The top 5 high-frequency Chinese medicines were Danggui(Radix Angelicae Sinensis),Chuanxiong(Rhizoma Chuanxiong),Honghua(Flos Carthami),Niuxi(Radix Achyranthis Bidentatae),and Taoren(Semen Persicae).The medical properties and flavors were mostly sweet,bitter,and warm,with the liver and spleen meridians being the main meridians.The association analysis suggested that Taoren,Honghua,Danggui,and Chuanxiong were the core Chinese medicines.Through cluster analysis,14 Chinese medicine combinations were obtained.Conclusion:The medication approach for postoperative pain in knee arthroplasty is to promote blood circulation and dredge meridians,and remove blood stasis and relieve pain in traditional Chinese medicine.Based on the results of syndrome differentiation and treatment,other Chinese medicines can be added or subtracted on the basis of the core prescription,providing a reference for future clinical treatment of the disease.
