摘 要:目的分析冠心病(CAD)合并2型糖尿病(T2DM)患者冠状动脉周围脂肪组织(PCAT)中的差异表达基因,筛选CAD合并T2DM的关键差异表达基因,并进行验证。方法在GEO数据库官网选取芯片GSE64554、GSE120774、GSE16415、GSE71416信息,筛选在CAD、T2DM均呈差异表达的基因,进行功能注释和通路分析,获得CAD合并T2DM的关键差异表达基因。选择行心脏瓣膜置换术的患者、CAD患者和CAD合并T2DM患者各20例分别纳入对照组、CAD组和CAD+T2DM组,选择术中留取的PCAT,采用RT-qPCR法检测三组PCAT中的关键差异表达基因。结果共得到CAD、T2DM患者PCAT中均呈差异表达基因54个,根据功能注释得到3个关键基因即人Ⅱ类主要组织相容性复合体DRβ1(HLA-DRβ1)、肿瘤蛋白53(TP53)、CC趋化因子受体9(CCR9)。对照组、CAD组、CAD+T2DM组TP53基因相对表达量依次增高(P均<0.05)。CAD组和CAD+T2DM组CCR9表达高于对照组(P均<0.05)。CAD+T2DM组HLA-DRβ1表达高于对照组(P<0.05)。结论CAD合并T2DM患者PCAT中关键差异表达基因为HLA-DRβ1、TP53和CCR9,其中TP53基因在CAD合并T2DM患者PCAT中表达变化显著,可能有助于CAD合并T2DM的诊断。Objective To analyze the differentially expressed genes in pericoronary adipose tissues(PCAT)of patients with coronary artery disease(CAD)combined with type 2 diabetes mellitus(T2DM),to screen the key differentially expressed genes of CAD and T2DM,and to validate the findings.Methods In the GEO database,the chip datasets GSE64554,GSE120774,GSE16415,and GSE71416 were obtained.Genes that were differentially expressed in both CAD and T2DM were screened out for functional annotation and pathway analysis to identify key genes associated with CAD and T2DM.Twenty patients undergoing heart valve replacement surgery,20 CAD patients,and 20 CAD patients with T2DM were included in the control group,CAD group,and CAD+T2DM group,respectively.PCAT samples collected during surgery were analyzed for key differentially expressed genes using RT-qPCR.Results A total of 54 differentially expressed genes were identified in the PCAT of CAD and T2DM patients.Functional annotation revealed three key genes:major histocompatibility complex class II DRβ1(HLA-DRβ1),tumour protein 53(TP53),and C-C motif chemokine receptor 9(CCR9).The relative expression levels of TP53 gene increased sequentially in the control group,CAD group,and CAD+T2DM group(all P<0.05).The expression of CCR9 in the CAD and CAD+T2DM groups was higher than that in the control group(both P<0.01).The expression of HLA-DRβ1 in the CAD+T2DM group was higher than in the control group(P<0.05).Conclusions The key genes in the PCAT of CAD patients with T2DM are HLA-DRβ1,TP53,and CCR9,with significant expression changes in the TP53 gene,which may contribute to the diagnosis of CAD complicated with T2DM.
