摘 要:急性肝衰竭(acute liver failure,ALF)是指短时间内发生的肝功能急剧受损而无法维持正常生理功能的一种危重疾病,其预后不良甚至死亡。肠道作为人体内最大的免疫器官之一对免疫系统具有重要调节作用,可作为治疗疾病及其并发症的突破口;良好的肠道功能对人体健康是十分有利的,通过正确的干预手段去保护肠道黏膜屏障、调节肠道菌群、清除肠道毒素、纠正免疫炎症紊乱等促进肠道微生物稳态是保障肠道功能的重要环节。肠道黏膜屏障-通透性改变、肠道菌群产物、免疫因子失调等导致的免疫炎症反应与ALF密切相关;NLRP3信号通路是炎症反应和免疫调节中的重要通路,在ALF的肝细胞焦亡、氧化应激、肝细胞纤维化及肠道微生态相关作用中扮演着重要角色。该文以中医“肝与大肠相通”理论为切入点,探讨NLRP3信号通路与急性肝衰竭肠道微生态的相关性及主要机制,为ALF的防治提供思路和参考。Acute liver failure(ALF)is a critical disease that occurs in a short period of time when the liver function is severely damaged and the normal physiological function cannot be maintained,and its prognosis is poor or even death.As one of the largest immune organs in the human body,the intestine plays an important role in regulating the immune system and can be used as a breakthrough in the treatment of diseases and their complications.Greating intestinal function is very beneficial to human health.Protecting intestinal mucosal barrier,regulating intestinal flora,removing intestinal toxins,correcting immune inflammatory disorders and promoting intestinal microbial homeostasis through correct interventions are important links to ensure intestinal function.The immune inflammatory response caused by intestinal mucosal barrier-permeability changes,intestinal flora products,and immune factor disorders is closely related to ALF.The NLRP3 signaling pathway is an important pathway in inflammatory response and immune regulation,and plays an important role in hepatocyte pyroptosis,oxidative stress,hepatocyte fibrosis and intestinal microecology in ALF.Based on the theory of“the liver and intestine interlinked”in traditional Chinese medicine,this paper discusses the correlation and main mechanism between NLRP3 signaling pathway and intestinal microecology of ALF,and provides ideas and references for the prevention and treatment of ALF.
