补肾生血汤联合免疫抑制治疗慢性再生障碍性贫血患者疗效及改善疾病相关指标研究

摘　　要：目的探讨补肾生血汤联合免疫抑制在慢性再生障碍性贫血患者中的应用。方法该研究纳入了76例慢性再生障碍性贫血患者作为研究对象,均为临汾市人民医院2019年3月—2022年3月期间收治的患者。依据随机数字表法将患者随机分为对照组和观察组,每组各38例。对照组采用常规治疗联合免疫抑制,观察组在对照组基础上加用补肾生血汤,均连续治疗3个月。对比两组治疗后临床疗效,治疗期间不良反应发生情况,治疗前后T淋巴细胞亚群水平、骨髓碱性成纤维细胞生长因子(bFGF)、微血管密度(MVD)、血管内皮生长因子(VEGF)、外周血象指标水平。结果治疗后观察组患者的临床总有效率为97.37%,明显高于对照组的84.21%;相较于治疗前,治疗后两组患者CD3+、CD4+、CD4+/CD8+、骨髓bFGF、MVD、VEGF、白细胞(WBC)、血红蛋白(Hb)、血小板(PLT)水平均升高,CD8+水平则降低,且观察组优于对照组;治疗期间,观察组患者的不良反应总发生率10.53%较对照组的28.95%相比更低(均P<0.05)。结论补肾生血汤联合免疫抑制治疗慢性再生障碍性贫血患者具有较好的临床疗效,能够改善患者机体内的免疫功能以及骨髓造血微环境,提升其造血功能,且安全性较高。Objective To explore the application of Bushen Shengxue Decoction(补肾生血汤)combined with immunosuppression in patients with chronic aplastic anemia.Methods This study included 76 patients with chronic aplastic anemia as the research sample,all of whom were admitted to Linfen People's Hospital between March 2019 and March 2022.The research subjects were divided into a control group and an observation group,with 38 cases in each group according to the random number table method.The control group received conventional treatment combined with immunosuppression,while the observation group received continuous treatment with Bushen Shengxue Decoction for 3 months on the basis of the above.Comparing the clinical efficacy of two groups after treatment,the occurrence of adverse reactions during treatment,the levels of T lymphocyte subsets,bone marrow basic fibroblast growth factor(bFGF),microvascular density(MVD),vascular endothelial growth factor(VEGF),and peripheral blood parameters before and after treatment.Results The results showed that the total clinical effective rate of the observation group after treatment was 97.37%,significantly higher than the control group's 84.21%;Compared with before treatment,the levels of CD3+,CD4+,CD4+/CD8+,bone marrow bFGF,MVD and VEGF,white blood cell(WBC),hemoglobin(Hb),and platelet(PLT)in both groups of patients increased after treatment.Comparing the post-treatment data between the observation group and the control group,the former was higher,while the CD8+level decreased.Comparing the post-treatment data between the observation group and the control group,the former was lower;During the treatment period,the total incidence of adverse reactions in the observation group was 10.53%,which was lower than the 28.95%in the control group(all P<0.05).Conclusion The combination of Bushen Shengxue Decoction and immunosuppressive therapy has a high clinical efficacy in the treatment of patients with chronic aplastic anemia.It can improve the immune function and bone marrow hematopoietic
