多巴胺结合硝普钠治疗老年重症心力衰竭的临床效果及对CysC、Hcy、BNP水平的影响

摘　　要：目的分析多巴胺结合硝普钠在老年重症心力衰竭(SHF)患者中的应用效果。方法选取2020年1月至2022年12月收治的106例老年SHF患者作为研究对象,根据治疗方案差异将其分为对照组(n=53,硝普钠)和研究组(n=53,多巴胺结合硝普钠)。比较两组的心功能指标、生化指标、心率变异性指标。结果治疗后,研究组的左心室射血分数(LVEF)、每搏量(SV)及每搏指数(SI)高于对照组(P<0.05)。治疗后,研究组的胱抑素C(CysC)、同型半胱氨酸(Hcy)、B型脑钠肽(BNP)、乏氧诱导因子-1α(HIF-1α)、嗜铬粒蛋白A(CGA)水平低于对照组(P<0.05)。治疗后,研究组的窦性心律RR间期的标准差、RR间期平均值标准差、相邻正常R-R间期差值、相邻NN之差>50 ms的个数占窦性心搏个数的百分比高于对照组(P<0.05)。结论多巴胺结合硝普钠治疗老年SHF患者有助于改善其心功能,调节生化指标,控制心率变异性,延缓病情进展。Objective To analyze the application effect of dopamine combined with sodium nitroprusside in elderly patients with severe heart failure(SHF).Methods A total of 106 elderly patients with SHF admitted from January 2020 to December 2022 were selected as the research objects.According to the difference of treatment plan,they were divided into control group(n=53,sodium nitroprusside)and study group(n=53,dopamine combined with sodium nitroprusside).The cardiac function indexes,biochemical indexes and heart rate variability indexes of the two groups were compared.Results After treatment,the left ventricular ejection fraction(LVEF),stroke volume(SV)and stroke index(SI)in the study group were higher than those in the control group(P<0.05).After treatment,the levels of cystatin C(CysC),homocysteine(Hcy),B-type natriuretic peptide(BNP),hypoxia inducible factor-1α(HIF-1α)and chromogranin A(CGA)in the study group were lower than those in the control group(P<0.05).After treatment,the standard deviation of RR interval of sinus rhythm,the standard deviation of mean value of RR interval,the difference of adjacent normal R-R interval and the percentage of the number of adjacent NN difference>50 ms in the number of sinus heart beats in the study group were higher than those in the control group(P<0.05).Conclusion Dopamine combined with sodium nitroprusside in the treatment of elderly patients with SHF is helpful to improve their cardiac function,regulate biochemical indexes,control heart rate variability and delay the progression of the disease.
