针药联合治疗奥沙利铂所致周围神经毒性的临床研究

摘　　要：目的探讨电针联合通络活血汤治疗奥沙利铂所致周围神经毒性(OIPN)的临床效果。方法选取2021年1月至2023年1月于空军军医大学第二附属医院中医科住院的90例OIPN患者作为研究对象,采用随机数字表法将其分为对照组(n=45)和观察组(n=45)。对照组给予甲钴胺治疗,观察组在对照组基础上采用电针联合通络活血汤。比较两组的治疗效果。结果观察组的治疗总有效率为80.00%,高于对照组的60.00%(P<0.05)。治疗后,观察组的OIPN等级优于对照组(P=0.023)。治疗后,两组的卡诺夫斯基健康状况量表(KPS)评分高于治疗前,且观察组高于对照组(P<0.05)。治疗后,两组的肢体麻木、关节疼痛、感觉迟钝评分低于治疗前,且观察组低于对照组(P<0.05)。两组治疗期间均未出现晕针、断针、血肿以及血常规、肝肾功能检测结果异常情况。结论电针联合通络活血汤治疗能够提高OIPN的临床效果,且安全性较高。Objective To explore the clinical effect of electroacupuncture combined with Tongluo Huoxue decoction in the treatment of oxaliplatin-induced peripheral neurotoxicity(OIPN).Methods A total of 90 patients with OIPN who were hospitalized in the traditional Chinese medicine department of the Second Affiliated Hospital of Air Force Military Medical University from January 2021 to January 2023 were selected as the research objects.The patients were divided into control group(n=45)and observation group(n=45)by random number table method.The control group was treated with mecobalamin,and the observation group was treated with electroacupuncture combined with Tongluo Huoxue decoction on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 80.00%,which was higher than 60.00%in the control group(P<0.05).After treatment,the OIPN grade of the observation group was better than that of the control group(P=0.023).After treatment,the Karnofsky Performance Scores(KPS)of the two groups was higher than that before treatment,and that in the observation group was higher than the control group(P<0.05).After treatment,the scores of limb numbness,joint pain and sensory retardation in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).During the treatment of the two groups,there were no dizzy needles,broken needles,hematoma,and abnormal blood routine,liver and kidney function test results.Conclusion Electroacupuncture combined with Tongluo Huoxue decoction can improve the clinical effect of OIPN,and has high safety.
