摘　　要：目的总结经外周静脉置入中心静脉导管(PICC)置管在高龄卧床患者中的应用与护理经验,旨在提高穿刺率,减轻因反复穿刺给患者带来的痛苦。方法回顾性分析2022年1月至2023年3月我院收治的90例高龄卧床患者的临床资料,分析高龄卧床患者PICC置管失败的原因,并提出相应护理对策。结果90例患者中,79例一次顺利置管成功,其余11例在置管过程中遇到一定问题(5例出现皮肤及血管问题,5例导管送入困难,1例导管异位),经调整后均置管成功。结论规范化操作、充分评估、合理选择置管路径、适宜的穿刺针以及恰当的体位,是提高高龄卧床患者PICC置管成功率的必要条件。Objective To summarize the application and nursing experience of peripherally inserted central catheter(PICC)catheterization in elderly bedridden patients,in order to improve the puncture rate and reduce the pain caused by repeated puncture.Methods The clinical data of 90 elderly bedridden patients admitted in our hospital from January 2022 to March 2023 were retrospectively analyzed.The reasons for the failure of PICC catheterization in elderly bedridden patients were analyzed,and the corresponding nursing countermeasures were put forward.Results Among the 90 patients,79 cases were successfully catheterized at one time,and the remaining 11 cases encountered certain problems in the process of catheterization(5 cases had skin and vascular problems,5 cases had difficulty in catheter insertion,and 1 case had ectopic catheter),after adjustment,the catheterization was successful.Conclusion Standardized operation,full evaluation,reasonable selection of catheterization path,appropriate puncture needle and appropriate position are the necessary conditions to improve the success rate of PICC catheterization in elderly bedridden patients.
