摘　　要：目的探讨应用一步式耻骨上导入器治疗儿童膀胱多发磁珠异物的疗效。方法回顾性分析2021年2月至2023年2月天津市儿童医院泌尿外科收治的4例膀胱多发磁珠异物患儿临床资料,4例患儿均应用一步式耻骨上导入器行膀胱异物取出术,术中行床旁X线检查确保患儿体内无磁珠残留。记录并分析取出磁珠的数量、手术时间、术中出血量、术后是否有并发症、术后住院时间、手术切口家长满意度。结果4例患儿均顺利一次手术取出异物,取出磁珠的数量分别为36颗、12颗、22颗、9颗,其中1例患儿磁珠周围形成包裹性结石,术中结石一并取出,经分析结石成分为六水合磷酸铵镁及尿酸铵。患儿手术时间为(30.0±9.1)min,范围为20~41 min;出血量为(3.0±0.8)ml,范围为2~4 ml,患儿术后平均住院时间为7 d。术后随访1~6个月,患儿无手术并发症发生,伤口美观,手术切口家长满意。结论应用一步式耻骨上导入器治疗儿童膀胱多发磁珠异物,操作简便,伤口美观,并发症少,值得临床推广应用。Objective To evaluate the efficacy of one-step suprapubic introducer for magnetic beads in bladder of children.Methods From February 2021 to February 2023,retrospective analysis of clinical data of 4 children hospitalized with magnetic beads in bladder.Bladder foreign body was removed by an one-step suprapubic introducer.And bedside intraoperative radiography was performed for ensuring that there were no residual magnetic beads in body.Count of removed beads,operative duration,intraoperative volume of blood loss,postoperative complications,postoperative hospital stay and parental satisfaction with surgical incision were analyzed.Results All procedures were completed successfully.And 36,12,22 and 9 magnetic beads were removed entirely.One case developed an encapsulated stone around magnetic beads and the stone was removed.And the stone was composed of magnesium ammonium phosphate hexahydrate and ammonium urate.Operative duration was(30.0±9.1)(20-41)min,hemorrhagic volume(3.0±0.8)(2-4)ml and average postoperative hospitalization stay 7 days.During a follow-up period of(1-6)months,there was no complication.All wounds healed well without incision infection.Parents were satisfied with surgical incision.Conclusions Satisfactory outcomes may be achieved for one-step suprapubic introducer removal of magnetic beads in bladder.And this mini-invasive procedure allows easier handling with an excellent efficacy.A wider clinical popularization is recommended.
