摘 要:目的:比较腕管综合征(CTS)患者正中神经远端拇指-腕点与中指-腕点感觉神经潜伏期(DSL)延长程度及传导速度(SCV)下降程度。方法:选取2022年9月—2023年4月在南京市江宁医院就诊的CTS患者42例,纳入观察组;选取同期健康体检者或无周围神经病史的其他意愿受检者31名,纳入对照组。检查并记录CTS患者患侧手及对照组任一侧手正中神经拇指-腕点、中指-腕点DSL、SCV结果,计算两组患者拇指-腕点、中指-腕点DSL、SCV的差值(ΔDSL、ΔSCV),比较两组间的差异。结果:观察组拇指-腕点DSL长于对照组(P<0.05);但两组中指-腕点DSL和两指间ΔDSL比较,差异无统计学意义(P>0.05);观察组拇指-腕点、中指-腕点SCV均慢于对照组,且两组ΔSCV差异有统计学意义(P<0.05)。结论:在CTS患者中检测正中神经远端拇指DSL及SCV较中指更敏感。Objective To compare the thumb-wrist and middle-finger-wrist distal sensory latency(DSL)and sensory nerve conduction velocity(SCV)of the median nerve in patients with carpal tunnel syndrome(CTS).Methods A total of 42 patients with CTS treated in Nanjing Jiangning Hospital from September 2022 to April 2023 were retrospectively selected and included in the observation group,and a total of 31 healthy subjects or other willing subjects without a history of peripheral nerve disease were retrospectively selected and included in control group.Examined diseased hand of observation group patients and any hand of the control group,then recorded thumb-wrist and middle finger-wrist DSL and SCV of every examined hands,calculated difference value of recorded DSL and SCV(ΔDSL,ΔSCV).Analyzed the statistical differences between the two groups.Results The thumb-wrist DSL in observation group was significantly longer than that in control group(P<0.05),but there was no significant difference in middle finger-wrist DSL andΔDSL(P>0.05).The SCV of thumb-wrist and middle finger-wrist in observation group were significantly slower than that in control group,andΔSCV was also statistically different from control group(P<0.05).Conclusions In patients with CTS,the sensory conduction latency and conduction velocity of median thumb nerve are more sensitive than those of middle finger.
