摘　　要：目的:观察自制硅胶管软垫加压包扎应用于桡动脉穿刺术后压迫止血的效果。方法:选取2021年3-5月江苏省昆山市中医医院带有创动脉血压监测管路装置转入麻醉恢复室的70例患者作为研究对象。根据不同加压包扎方法分为观察组(n=35)和对照组(n=35),对照组采用传统自制纺纱块压迫止血法,观察组在桡动脉导管拔出时使用自制硅胶管软垫作为内衬,并采用3层“蝶型扣”纱布包扎法,两组患者均加压包扎30 min。观察两组皮下出血和血肿、术肢肿胀及远端血液循环情况;观察组患者去除包扎后皮肤压痕与穿刺点及桡动脉走向的关系。结果:观察组35例患者均不同程度显现硅胶管压痕;3例穿刺点位于硅胶管压痕中心位置,其余32例穿刺点位于硅胶管压痕的远心端1/3内;23例硅胶管压痕与桡动脉走向吻合、5例平行、7例交叉。观察组在减少皮下出血及皮下血肿、减轻远端肢体肿胀和血液回流障碍方面均优于对照组,差异有统计学意义(P<0.05)。结论:自制硅胶管软垫结合3层“蝶型扣”纱布包扎法能够提高桡动脉穿刺术后压迫止血效果,减少对远端肢体血液回流的影响,减少复苏室护士工作量,提高患者的舒适度。Objective To observe the effect of self-made silicone tube cushion compression bandage applied to compression hemostasis after radial artery puncture.Methods From March to May 2021,a total of 70 patients with invasive arterial blood pressure monitoring pipeline device transferred to anesthesia recovery room in Kunshan Hospital of Traditional Chinese Medicine,Jiangsu Province were selected as the research objects.They were divided into the observation group(n=35)and the control group(n=35)by different pressure dressing methods.The control group used the traditional compression hemostasis method with self-made spinning gauze block,the observation group used the self-made silicone tube cushion as the lining when the radial artery catheter was pulled out,and used the gauze bandaging method of three layers of"butterfly buckle".The patients in both groups were pressurized and bandaged for 30 minutes.The subcutaneous hemorrhage and hematoma,limb swelling and distal blood circulation were observed in the two groups;the relationship between silicone tube indentation with puncture point and radial artery direction after removing bandage in the observation group also be observed.Results In the observation group,35 patients showed different degrees of silicone tube indentation;the puncture points of 3 patients were located in the center of the silicone tube indentation,and the puncture points of the other 32 patients were located in 1/3 of the distal end of the silicone tube indentation;the silicone tube indentation was anastomosed with the radial artery in 23 cases,and parallel with the radial artery in 5 cases,crossed with the radial artery in 7 cases.The observation group was significantly better than the control group in reducing subcutaneous hemorrhage and hematoma,reducing distal limb swelling and blood reflux disorder(P<0.05).Conclusions The gauze dressing method of self-made silicone tube cushion combined with 3 layers of"butterfly buckle"can improve the hemostatic effect of compression after radial artery puncture,r
