某院药房拆零药品质量控制管理探索

摘　　要：目的提升医院药房拆零药品质量控制(简称质控)管理水平,保障患者用药安全。方法针对某院拆零药品质控现存风险点,从药品(包括库存、效期与批号、滞销药品、裸片或胶囊识别)、人员、环境、设备、制度5个方面制订管理措施。收集医院信息系统中该院2022年7月至12月(管理措施实施前)、2023年1月至6月(管理措施实施后)的拆零药品信息,并进行比较。结果实施后,该院拆零药品质控检查评分由(71.00±2.00)分升至(95.67±2.28)分(P<0.05);拆零药品月均账物相符率由(76.83±0.98)%升至(85.33±4.27)%,月均库存周转天数由(16.50±0.53)d缩短至(14.07±1.17)d,月均挤压破损量由(81.67±16.11)片减至(52.00±7.46)片,调剂效率由(65.33±5.01)包/min升至(89.33±1.75)包/min(P<0.05);月均分包差错(装量错误、品种错误、批号错误)发生次数及药品异常(外观变化、质量变化、微生物污染)发生品种数均显著减少(P<0.05);医护人员对药房拆零药品的调剂效率、标识清晰度、质控管理的满意度均显著提升(P<0.05)。结论实施的质控管理对策有效提高了医院拆零药品质控管理水平,增强了拆零药品管理的规范性和系统性。Objective patients'medication safety.Methods Based on the existing risk points for quality control of dismantled drugs in a hospital,the management measures were formulated from drugs(including inventory,expiration date and batch number,unsold drugs,identification of naked tablets or capsules),personnel,environment,equipment and system.The information of dismantled drugs in the hospital from July to December 2022(before the implementation of management measures)and from January to June 2023(after the implementation of management measures)was collected from the hospital information system,and the relevant data were compared.Results After implementation,the quality control inspection score of the dismantled drugs in the hospital increased from(71.00±2.00)points to(95.67±2.28)points(P<0.05);the monthly consistency rate of accounts and materials of dismantled drugs increased from(76.83±0.98)%to(85.33±4.27)%,the monthly inventory turnover time shortened from(16.50±0.53)d to(14.07±1.17)d,the monthly compression damage amount decreased from(81.67±16.11)tablets to(52.00±7.46)tablets,the dispensing efficiency increased from(65.33±5.01)bags/min to(89.33±1.75)bags/min(P<0.05);the monthly incidence of packaging errors(incorrect deliverable volume,variety,batch number)and abnormal varieties(appearance changes,mass changes,microbial contamination)significantly decreased(P<0.05);the satisfaction level of medical staff with the dispensing efficiency,clarity of labeling,and quality control management of dismantled drugs in pharmacy significantly improved(P<0.05).Conclusion The implemented quality control management measures have effectively promoted the quality control management of dismantled drugs in hospital,improved the standardization and systematicity of dismantled drug management.
