内镜下电凝切除术与内镜下黏膜切除术治疗肠道息肉的效果比较

摘　　要：目的比较内镜下电凝切除术与内镜下黏膜切除术治疗肠道息肉的临床效果。方法队列研究。抽取2021年5月至2023年12月濮阳市安阳地区医院收治的肠道息肉患者91例,按随机数字表法分为对照组(45例)和观察组(46例)。对照组行内镜下电凝切除术治疗,观察组行内镜下黏膜切除术治疗。比较两组围术期情况(手术时间、术中出血量、息肉切除时间、术后首次进流食时间、住院时间)、炎症因子[降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素-6(IL-6)及白细胞介素-17(IL-17)]水平、术后并发症和复发情况。结果两组患者住院时间比较差异未见统计学意义(P>0.05)。观察组术中出血量少于对照组(P<0.05);观察组手术、息肉切除及术后首次进流食时间均短于对照组(P均<0.05)。术后,观察组PCT、CRP、IL-6、IL-17水平均低于对照组(P均<0.05)。随访6个月后,观察组并发症发生率和复发率均低于对照组(P均<0.05)。结论与内镜下电凝切除术相比,内镜下黏膜切除术在肠道息肉患者中的治疗效果更好,炎症反应更轻,术后并发症发生率和复发率更低。ObjectiveTo compare the clinical effects of endoscopic electrocoagulation resection and endoscopic mucosal resection in the treatment of intestinal polyps.MethodsA total of 91 patients with intestinal polyps treated in Anyang District Hospital of Puyang City from May 2021 to December 2023 were selected for the cohort study,and they were divided into a control group(45 cases)and an observation group(46 cases)by random number table method.The control group underwent endoscopic electrocoagulation resection,while the observation group underwent endoscopic mucosal resection.The perioperative conditions,including operation duration,intraoperative blood loss,polyps resection time postoperative first feeding time,and hospital stay,levels of inflammatory factors,including procalcitonin(PCT),C-reactive protein(CRP),interleukin-6(IL-6)and interleukin-17(IL-17),the postoperative complications and recurrence rate were compared between the two groups.ResultsThere was no significant difference in hospital stay between the the two groups(P>0.05).The intraoperative bleeding volume in the observation group was lower than that in the control group(P<0.05).The operation duration,polypectomy time and time for first liquid diet after operation in the observation group were shorter than those in the control group(all P<0.05).After operation,the levels of PCT,CRP,IL-6 and IL-17 in the observation group were lower than those in the control group(all P<0.05).After 6 months of follow-up,the incidence of complications and recurrence rate in the observation group were lower than those in the control group(all P<0.05).ConclusionsCompared with endoscopic electrocoagulation resection,endoscopic mucosal resection is more effective in the treatment of intestinal polyps,which can decrease inflammatory response,reduce postoperative complications and lower recurrence rate.
