摘　　要：目的基于ADOPT模式,了解老年肛周疾病术后患者的自我保护需求。方法采用目的抽样法,选取2023年10月至2024年4月中国人民解放军第三〇五医院肛肠外科收治的老年肛周疾病手术患者12例为研究对象,对患者进行半结构式访谈,运用Colaizzi七步法分析资料,提炼主题。结果提炼出老年肛周疾病术后患者自我保护需求的5个主题:手术意愿强但自我保护意识不足(态度),自我保护知识缺乏(定义),资源利用不完善(开放思维),自我保护主动性较低(计划),自我保护执行程度低(实施)。结论应以老年肛周疾病术后患者自我保护需求特点为导向,开展围术期宣教,制定疾病知识、护理方法、辅具使用等相结合的综合性、个性化的自我保护方案。Objective To investigate the self-preservation needs of elderly patients after perianal surgery based on ADOPT model.Methods From October 2023 to April 2024,12 elderly surgery patients admitted to the anorectal surgery department of the 305 Hospital of PLA were selected by purpose sampling method.Semi-structured interviews were conducted with patients.The data was analyzed using Colaizzi method.Results Five themes of self-preservation needs of elderly patients after perianal surgery were extracted:strong willingness to surgery but insufficient awareness of self-preservation(attitude),lack of self-preservation knowledge(definition),imperfect utilization of resources(open mind),low initiative of self-preservation(plan),low degree of self-preservation implementation(try it out).Conclusions Based on the characteristics of self-preservation needs of elderly patients with perianal surgery,we should carry out perioperative propaganda and education,and formulate a comprehensive and personalized self-preservation plan combining disease knowledge,nursing methods and the use of assistive devices.
