摘 要:目的比较胎儿系统超声和常规产科超声对妊娠期胎儿畸形的诊断价值。方法选取2020年1月至2023年6月在宜春市妇幼保健院进行孕前常规检查的70例中晚期孕妇作为研究对象。所有孕妇均进行常规产科超声、胎儿系统超声检查,金标准为引产结果或出生后证实结果,统计并比较两种超声方式的具体胎儿畸形类型检出率、诊断结果、诊断效能。结果系统超声检查的胎儿畸形类型检出率(90.00%)高于常规产科检查(20.00%),差异有统计学意义(P<0.05);常规产科超声检查、系统超声检查的真阳性例数分别为2例、9例,系统超声检查的敏感度、特异度、准确度、阳性预测值、阴性预测值数据分别为90.00%、96.67%、95.71%、81.82%、98.31%,均高于常规产科检查的20.00%、93.00%、82.86%、33.33%、87.50%。结论同常规产科超声检查进行比较,胎儿系统超声在检出妊娠期尤其是中晚期胎儿畸形方面效果更优,具有较高的临床推广应用价值。Objective To compare the diagnostic value of fetal system ultrasound and conventional obstetric ultrasound for fetal malformations during pregnancy.Methods A total of 70 middle and late pregnant women who underwent pre-pregnancy routine examination in Yichun Maternal and Child Health Hospital from January 2020 to June 2023 were selected as the research objects.The gold standard was the results of induced labor or confirmed after birth.The detection rate of specific fetal malformation types,diagnostic results,and diagnostic efficiency of the two ultrasound methods were calculated and compared.Results The detection rate of fetal malformation types by systematic ultrasound examination(90.00%)was higher than that by conventional obstetrical examination(20.00%),and the difference was statistically significant(P<0.05).The true positive cases of conventional ultrasound and systematic ultrasound were 2 cases and 9 cases,respectively.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of systematic ultrasound were 90.00%,96.67%,95.71%,81.82% and 98.31%,respectively.All of them were higher than those of conventional obstetric examination(20.00%,93.00%,82.86%,33.33%,87.50%).Conclusion Compared with conventional obstetric ultrasound,fetal systematic ultrasound has better effect in detecting fetal malformations during pregnancy,especially in the middle and late stages,and has high clinical application value.
