摘 要:目的对超声剪切波弹性成像(SWE)在肾脏占位性病变良恶性中的诊断及应用价值进行探讨。方法回顾性分析2021年1月至2023年12月福建中医药大学附属第三人民医院收治的82例肾占位患者的临床资料,所有患者均接受超声剪切波弹性成像检查,并经组织病理学检查证实为肾脏占位性病变。以病理检查结果为金标准,分为肾良性病变组(48例)及恶性病变组(34例),比较两组一般临床资料及SWE影像资料,并分析SWE在肾脏占位性病变良恶性中的诊断价值。结果SWE诊断肾占位病变良性检出率为91.67%(44/48),恶性检出率为82.35%(28/34),与病理结果一致性较好(Kappa=0.769);恶性病变组双肾杨氏模量衡量值(E_(mean))、最大值(E_(max))、最小值(Emin)均高于良性病变组,差异有统计学意义(P<0.05);E_(mean)、E_(max)及Emin联合检测肾脏占位性病变的曲线下面积为0.941,均高于三者单一检测(P<0.05)。结论SWE可以无创的诊断患者肾脏占位性病变的良恶性并提高诊断准确度,诊断效能较好,有一定临床应用价值。Objective To discuss the diagnostic value and application of ultrasonic shear wave elastography(SWE)in benign and malignant renal space occupying lesions.Methods The clinical data of 82 patients with renal space occupying admitted to the Third Affiliated People's Hospital of Fujian University of Traditional Chinese Medicine from January 2021 to December 2023 were retrospectively analyzed.All patients underwent ultrasonic shear wave elastography and confirmed by histopathological examination as renal space occupying lesions.With the pathological examination results as the gold standard,they were divided into the benign lesion group(48 cases)and malignant lesion group(34 cases).General clinical data and SWE imaging data of the two groups were compared,and the diagnostic value of SWE in benign and malignant renal space-occupying lesions was analyzed.Results The positive rate of benign and malignant lesions diagnosed by SWE was 91.67%(44/48)and 82.35%(28/34),which were in good agreement with the pathological results(Kappa=0.769).The mean Young's modulus(E_(mean)),maximum Young's modulus(E_(max))and minimum Young's modulus(Emin)of both kidneys in the malignant lesion group were higher than those in the benign lesion group,and the differences were statistically significant(P<0.05).The area under the curve of the combined detection of E_(mean),E_(max) and Emin was 0.941,which was higher than that of the three single detection(P<0.05).Conclusion SWE can noninvasively diagnose the benign and malignant renal space occupying lesions and improve the diagnostic accuracy.The diagnostic efficacy is good,and it has certain clinical application value.
