肝脓肿合并巨大肝脏炎性假瘤1例

摘　　要：自1953年首次报道肝脏炎性假瘤以来,该疾病的报道在不断增加,但罕见合并巨大细菌性肝脓肿(PLA)病例。肝脏炎性假瘤及肝脓肿均为肝脏炎症性病变,前者含炎性细胞浸润、结缔组织增生,后者含肝脏炎性局灶坏死病变,均较难通过无创性检查与其他肝脏良、恶性肿瘤相鉴别,且二者之间通过影像学检查同样难以鉴别。肝脏炎性假瘤在众多肝脏良性肿瘤仅占约5%,较为少见,且目前病因尚未明确。而其大多病例最终以病理结果才得以诊断明确,误诊率高达70%以上,给患者治疗带来一定负担。肝脏炎性假瘤合并肝脓肿的患者更为罕见,且直径>10 cm罕有报道,而两种疾病治疗过程中存在治疗方案的差异。本文通过分析该病例特点结合文献研读对该疾病综述和讨论,分析总结该类疾病相关特点,优化该疾病的诊疗过程,提供更早更优化的治疗方案。Since the first report of inflammatory pseudotumor of liver in 1953,the number of reports on this disease has been increasing,but rare cases of concomitant massive pyogenic liver abscesses(PLA)have been reported.Hepatitis pseudotumor and liver abscess are both inflammatory lesions of the liver.The former contains inflammatory cell infiltration and connective tissue proliferation,while the latter contains inflammatory focal necrosis lesions of the liver.Both are difficult to distinguish from other benign and malignant liver tumors through non-invasive examination,and it is also difficult to distinguish between the two through imaging examination.Hepatitis pseudotumor accounts for only about 5% of many benign liver tumors,which is relatively rare,and the etiology is currently unclear.And most of its cases are ultimately diagnosed based on pathological results,with a misdiagnosis rate of over 70%,which brings a certain burden to the treatment of patients.Patients with hepatitis pseudotumor combined with liver abscess are even rarer,and there are few reports of diameter>10 cm,and there are differences in treatment plans between the two diseases.This article reviews and discusses the characteristics of this case combined with literature review,summarizes the relevant characteristics of this type of disease,optimizes the diagnosis and treatment process of this disease,and provides earlier and more optimized treatment plans.
