摘  要:目的研究白细胞计数(WBC)、白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)、中性粒细胞与淋巴细胞比值(NLR)、淀粉样蛋白A(SAA)在细菌性支气管肺炎患儿诊断中的应用价值。方法选取2021年1月至2023年12月广西壮族自治区江滨医院100例住院的细菌性支气管肺炎患儿作为研究对象,作为观察组,以同期100名健康体检儿童作为对照组。比较两组患儿WBC、IL-6、hs-CRP、PCT、NLR、SAA水平,分析WBC、IL-6、hs-CRP、PCT、NLR、SAA对细菌性支气管肺炎的诊断价值;建立三指标联合检测模型,探讨各联合检测模型对细菌性支气管肺炎的诊断效能。结果观察组WBC、IL-6、hs-CRP、PCT、NLR、SAA水平均高于对照组,差异有统计学意义(P<0.05);WBC、IL-6、hs-CRP、PCT、NLR、SAA的ROC曲线下面积(AUC)分别为9.060、0.683、0.743、0.973、0.608、0.784;六指标联合检测模型(AUC=0.989),WBC+NLR+PCT三指标联合检测模型(AUC=0.984)、WBC+NLR+hs-CRP三指标联合检测模型(AUC=0.942)、WBC+NLR+IL-6三指标联合检测模型(AUC=0.931)、WBC+NLR+SAA三指标联合检测模型(AUC=0.927)。结论WBC、IL-6、hs-CRP、PCT、NLR、SAA对细菌性支气管肺炎均存在一定的诊断价值,其中六指标联合检测模型诊断效能最佳,WBC+NLR+PCT三指标联合检测模型诊断效能高且经济实惠。Objective To investigate the application value of white blood cell count(WBC),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),neutrophil-to-lymphocyte ratio(NLR),and amyloid A(SAA)in the diagnosis of bacterial bronchopneumonia in pediatric patients.Methods A total of 100 cases of hospitalized pediatric patients with bacterial bronchopneumonia from January 2021 to December 2023 in Jiangbin Hospital,Guangxi Zhuang Autonomous Region were selected as the research objects,and they were as selected as the observation group,and 100 healthy children during the same period were selected as the control group.The levels of WBC,IL-6,hs-CRP,PCT,NLR,and SAA were compared between the pediatric patients of the two groups,and the diagnostic value of WBC,IL-6,hs-CRP,PCT,NLR,and SAA for bacterial bronchopneumonia was analyzed.An optimal three-indicator combined detection model was established to investigate the diagnostic efficacy of each combined detection model for bacterial bronchopneumonia.Results The levels of WBC,IL-6,hs-CRP,PCT,NLR and SAA in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).The areas under the ROC curves(AUC)for WBC,IL-6,hs-CRP,PCT,NLR,SAA were as follows:9.060,0.683,0.743,0.973,0.608,0.784.Six indexes combined detection model(AUC=0.989),WBC+NLR+PCT three-index combined detection model(AUC=0.984),WBC+NLR+hs-CRP three-index combined detection model(AUC=0.942),WBC+NLR+IL-6 three-index combined detection model(AUC=0.931),WBC+NLR+SAA three-index combined detection model(AUC=0.927).Conclusion WBC,IL-6,hs-CRP,PCT,NLR and SAA all have certain diagnostic value in the diagnosis of bacterial bronchoppneumonia,among which the six indexes combined detection model has the best diagnostic efficiency,and the WBC+NLR+PCT combined detection model has high diagnostic efficiency and is economical.
