儿童荨麻疹患者血清25(OH)D、IgE水平与UAS评分的相关性研究

摘　　要：目的:探讨儿童荨麻疹血清25-羟基维生素D[25-hydroxy vitaminD,25(OH)D]、免疫球蛋白E(immunoglobulin E,IgE)水平与荨麻疹活动性评分,(UAS评分)之间的关系。方法:测定55例荨麻疹患儿及55例健康对照儿童外周血25(OH)D、总IgE水平,同时对荨麻疹患儿进行UAS评估,采用受试者工作曲线(receiver operating characteristic curve,ROC)评估血清25(OH)D及总IgE判断荨麻疹病情严重程度的效能。结果:中重度荨麻疹患儿外周血25(OH)D水平较对照组低56.1%,总IgE水平较对照组高97.1%(P<0.05)。Spearman相关分析提示,中重度组荨麻疹患儿UAS评分与血清25(OH)D水平呈负相关(P<0.01),与血清总IgE水平呈正相关(P<0.01)。受试者工作曲线分析显示,中重度荨麻疹患儿血清25(OH)D及总IgE水平灵敏度较高(P<0.01)。结论:血清25(OH)D可以辅助诊断儿童荨麻疹并评估病情进展。Objective:To explore the relationship between serum 25-hydroxyvitamin D[25(OH)D]and immunoglobulin E(IgE)levels and urticaria activity score(UAS score)in children with urticaria.Methods:The levels of 25(OH)D and total IgE in the peripheral blood were determined in 55 children with urticaria and 55 healthy controls.Meanwhile,UAS was evaluated for children with urticaria.The receiver operating characteristic(ROC)curve was used to evaluate the efficiency of serum 25(OH)D and total IgE in judging the severity of urticaria.Results:Peripheral blood 25(OH)D level in children with moderate-to-severe urticaria was lower than that in the control group while total IgE was higher than that in the control group(P<0.05).Spearman correlation analysis indicated that the UAS score of children with urticaria in the moderate-to-severe group was negatively correlated with serum 25(OH)D level(P<0.01),and was positively correlated with serum total IgE level(P<0.01).ROC curve analysis showed that the sensitivities of serum 25(OH)D and total IgE in children with moderate-to-severe urticaria were relatively high(P<0.01).Conclusion:Serum 25(OH)D can sist in the diagnosis of urticaria in children and asss the disease progression.
