国医大师潘敏求基于“郁”“毒”“虚”运用舒瘀消疬方辨治甲状腺癌的经验

摘　　要：总结潘敏求教授基于“郁”“毒”“虚”运用舒瘀消疬方辨治甲状腺癌的临证经验。潘敏求教授认为甲状腺癌的病因病机主要为“郁”“毒”“虚”相互作用,临床中总结出气滞痰凝、肝郁化火、痰结血瘀、气阴两虚4个证型,并以此病机创制辨病治疗甲状腺癌专方“舒瘀消疬方”以化痰散结、破血逐瘀、补脾疏肝,本方一方面强调扶助不足之正气,另一方面亦兼顾祛除肿块实邪。潘敏求教授在临证中以舒瘀消疬方为主方,强调要随时注意病证的发展变化,根据脉证的变化随证治之,灵活加减化裁,在临床上取得较好的疗效。This paper is intended to summarize the clinical experience of Professor PAN Minqiu in treating thyroid cancer with Shuyu Xiaoli Formula(SYXLF)based on"depression","toxicity",and"deficiency".Professor PAN believes that the etiology and pathogenesis of thyroid cancer is mainly due to interaction of"depression","toxicity",and"deficiency".In clinical practice,he has summarized four patterns:qi stagnation with phlegm retention,liver qi stagnation transforming into fire,phlegm retention with blood stagnation,and qi and yin deficiency pattern.Furthermore,based on the pathogenesis,he has created a specialized formula,SYXLF,for differentiating and treating thyroid cancer.This formula aims to transform phlegm and dissipate nodules,circulate blood and eliminate stasis,and strengthen the spleen and soothe the liver.On one hand,it emphasizes tonifying the deficient healthy qi;on the other hand,it also takes into account eliminating the solid masses and pathogenic factors.In his clinical practice,Professor PAN emphasizes the use of SYXLF as the primary one and underscores that attention should be paid to constantly monitoring the development and changes of the disease,and adjusting the treatment flexibly according to changes in pulse and symptoms.By making flexible modification to the formula,he has achieved good clinical efficacy.
