腹腔镜手术治疗肝内外胆管结石的临床研究

摘　　要：目的探讨肝内外胆管结石患者应用腹腔镜手术治疗的临床效果。方法选取68例肝内外胆管结石患者,将其随机分为对照组和观察组,每组34例。对照组采用开腹手术治疗,观察组采用腹腔镜手术治疗。对比两组患者的治疗效果、围术期指标及并发症发生率。结果观察组的治疗总有效率94.12%高于对照组的70.59%,差异具有统计学意义(P<0.05)。观察组患者术中出血量(173.74±31.26)ml、手术时间(127.45±30.09)min、切口长度(5.02±0.86)cm、住院时间(7.84±2.16)d均优于对照组的(215.39±35.22)ml、(149.98±28.42)min、(10.96±2.16)cm、(11.59±2.92)d,差异具有统计学意义(P<0.05)。观察组并发症发生率(5.88%)显著低于对照组(38.24%),差异具有统计学意义(P<0.05)。结论肝内外胆管结石患者应用腹腔镜手术治疗能够提高治疗效果,减少并发症发生,且术中出血量少,手术时间及住院时间短,值得在临床上大力推广与普及。Objective To explore the clinical effect of laparoscopic surgery for the treatment of intrahepatic and extrahepatic bile duct stones.Methods 68 patients with intrahepatic and extrahepatic bile duct stones were selected and randomly divided into the control group and the observation group,with 34 cases in each group.The control group was treated with open surgery,while the observation group was treated with laparoscopic surgery.The treatment effect,perioperative indicators,and incidence of complications were compared between the two groups of patients.Results The total effective rate of 94.12%in the observation group was higher than 70.59%in the control group,and the difference was statistically significant(P<0.05).In the observation group,the intraoperative blood loss was(173.74±31.26)ml,the operation time was(127.45±30.09)min,the incision length was(5.02±0.86)cm,and the hospital stay was(7.84±2.16)d,which were better than(215.39±35.22)ml,(149.98±28.42)min,(10.96±2.16)cm,and(11.59±2.92)d in the control group,and the difference was statistically significant(P<0.05).The complication rate of the observation group(5.88%)was significantly lower than that of the control group(38.24%),and the difference was statistically significant(P<0.05).Conclusion The application of laparoscopic surgery in the clinical treatment of patients with intrahepatic and extrahepatic bile duct stones can improve the treatment effect,reduce complications,with less intraoperative blood loss,shorter operation time and hospital stay.It is worth vigorously promoting and popularizing in clinical practice.
