活血膏外敷治疗血栓性外痔(气滞血瘀证)的临床研究

摘　　要：目的观察活血膏局部外敷治疗血栓性外痔(气滞血瘀证)的临床疗效。方法患者60例按随机数字表法分为两组各30例。两组患者均口服地奥司明片作为基础治疗,其中试验组患者在基础治疗上加用活血膏外敷治疗。观察两组患者总体疗效、治疗后的痔体血栓大小、疼痛、局部皮肤颜色、肛门坠胀异物感、D-二聚体等指标。结果试验组临床总有效率为93.33%,高于对照组的76.67%(P<0.05),痔体血栓大小、疼痛、局部皮肤颜色、肛门坠胀异物感评分均优于对照组;在改善D-二聚体水平上两组差异无统计学意义(P>0.05)。结论活血膏外敷治疗血栓性外痔(气滞血瘀证)在改善痔体血栓大小、疼痛、局部皮肤颜色、肛门坠胀异物感方面具有良好的临床疗效,切合病机,协同增效,缓解症状,避免进一步发展及手术治疗,有较好临床应用价值。Objective:To investigate the clinical efficacy of topical application of Huoxue Ointment in treating thrombotic external hemorrhoids(syndrome of qi stagnation and blood stasis).Methods:According to the predefined inclusion and exclusion criteria,a total of 60 patients diagnosed with thrombotic external hemorrhoids(syndrome of qi stagnation and blood stasis)were randomly assigned into two groups.Both groups received oral administration of diosmin tablets as the fundamental treatment,while the experimental group additionally underwent topical application of Huoxue Ointment.The overall efficacy,hemorrhoid thrombus size,pain,local skin color,anal bulge foreign body sensation and D-dimer were observed after treatment.Results:The clinical overall effective rate in the experimental group was 93.33%,and 76.67%in the control group(P<0.05).The scores for the size of thrombosed hemorrhoids,pain,local skin color,and the sensation of anal heaviness and foreign body were all better in the experimental group than in the control group.There was no significant difference between the two groups in terms of improving D-dimer levels(P>0.05).Conclusion:Topical application of Huoxue Ointment for treating thrombosed external hemorrhoids(syndrome of qi stagnation and blood stasis)demonstrates good clinical efficacy in improving the size of the thrombosed hemorrhoid,pain,local skin color,and the sensation of anal heaviness and foreign body,which aligns with the pathogenesis,synergizes effects,alleviates symptoms,prevents further development and surgical intervention,and has good clinical application value.
