摘 要:[目的]采用荟萃分析方法评价膝关节内注射脂肪源性血管基质组份(stromal vascular fraction,SVF)治疗膝关节骨性关节炎(knee osteoarthritis,KOA)的临床疗效。[方法]检索2022年7月29日之前在PubMed、Web of Science发表的相关文章,阅读全文提取效应量,采用STATA 14.2软件进行荟萃分析。[结果]从检索到的176篇文献中选出12篇,共401例患者,荟萃分析结果显示,与治疗前相比,治疗后VAS评分(MD=-2.944,95%CI-3.295~-2.539,P<0.001)、随访时间不足12个月WOMAC评分(MD=-16.676,95%CI-17.536~-15.817,P<0.001)、术后18个月WOMAC评分(MD=-32.940,95%CI-34.563~-31.317,P<0.001)均显著降低。无严重不良事件发生。[结论]基于目前可用的有限证据,SVF注射能在短期内降低KOA患者的VAS和WOMAC评分,提示其对KOA患者的疼痛及关节功能障碍有确切疗效,同时无严重不良事件。[Objective]To evaluate the clinical efficacy of intraaticular injection of adipose-derived stromal vascular fraction(SVF)in the treatment of knee osteoarthritis(KOA)by meta-analysis.[Methods]The relevant articles published in PubMed and Web of Science before July 29,2022 were retrieved,and data were extracted after reading the full text.STATA 14.2 software was used for meta-analysis.[Results]This meta-analysis included 12 out of 176 retrieved articles with a total of 401 patients involved.As results of the analysis,the VAS score after treatment(MD=-2.944,95%CI-3.295~-2.539,P<0.001),the WOMAC score with follow-up less than 12 months(MD=-16.676,95%CI-17.536 to -15.817),the WOMAC score(MD=-32.940,95%CI-34.563~-31.317,P<0.001)at 18 months after the operation was significantly decreased compared with those before treatment.No serious adverse events repored in any papers.[Conclusion]Based on the limited evidence currently available,SVF injection can reduce VAS and WOMAC scores in patients with KOA in the short term,suggesting that it is effective for pain and joint dysfunction in patients with KOA without serious adverse events.
