摘 要:目的分析系统性红斑狼疮患者血清转化生长因子β1(transforming growth factor-β1,TGF-β1)、白细胞介素4(interleukin-4,IL-4)、干扰素γ(interferon-γ,IFN-γ)水平变化及临床意义。方法选取2018年7月至2020年7月在南京医科大学附属泰州人民医院进行诊治的90例系统性红斑狼疮患者为研究对象,按照系统性红斑狼疮病情活动度评分(systemic lupus erythematosus activity index,SLEDAI)将其分为活动组(42例,SLEDAI≥10分)、稳定组(48例,SLEDAI<10分),再选取同期在该院进行健康体检的30例人群作为对照组,开展回顾性研究。比较三组人群血清TGF-β1、IL-4、IFN-γ水平,并分析其表达与病情活动度的关系。结果与活动组相比,稳定组、对照组血清TGF-β1水平明显升高,血清IL-4、IFN-γ水平明显降低,且活动组SLEDAI评分高于稳定组,差异有统计学意义[pg/mL:(648.35±24.35)比(1127.73±32.02)比(1422.39±38.84);(64.73±4.68)比(34.21±3.54)比(7.36±1.45);(79.90±5.27)比(54.66±3.86)比(16.38±1.66);分:(20.56±2.43)比(5.14±1.71),F值分别为5633.10、2224.37、2153.94、35.14,P值均<0.05]。Pearson分析表明,血清TGF-β1水平与SLEDAI评分呈负相关(r=-0.802,P<0.05),血清IL-4、IFN-γ水平与SLEDAI评分之间呈正相关(r=0.825、0.813,P<0.05)。受试者工作特征曲线分析结果显示,血清TGF-β1、IL-4、IFN-γ水平预测系统性红斑狼疮病情活动度的曲线下面积为0.872、0.892、0.905,95%CI为0.785~0.933、0.809~0.948、0.825~0.957,预测效能较好。结论系统性红斑狼疮患者血清TGF-β1水平明显降低、血清IL-4、IFN-γ水平明显升高,其异常表达与疾病发生存在一定联系,同时能够反映病情活动度。ObjectiveTo analyze the changes in serum levels of transforming growth factorβ1(TGF-β1),interleukin-4(IL-4)and interferon-γ(IFN-γ)in patients with systemic lupus erythematosus and their clinical significance.MethodsTotal of 90 patients with systemic lupus erythematosus who were diagnosed and treated in Taizhou People's Hospital affiliated with Nanjing Medical University from July 2018 to July 2020 were selected as the study subjects.They were divided into active group(42 cases,SLEDAI≥10 points),stable group(48 cases,SLEDAI<10 points)according to the systemic lupus erythematosus activity index(SLEDAI)score,and then 30 people who underwent physical examination in the hospital during the same period were selected as the control group for retrospective study.Compared the serum TGF-β1,IL-4 and IFN-γlevels of the three groups and analyzed the relationship between their expression and disease activity.ResultsCompared to the activity group,the levels of serum TGF-β1 in stable group and control group were significantly increased,while the levels of serum IL-4,IFN-γlevels were reduced[pg/mL:(648.35±24.35)vs(1127.73±32.02)vs(1422.39±38.84);(64.73±4.68)vs(34.21±3.54)vs(7.36±1.45);(79.90±5.27)vs(54.66±3.86)vs(16.38±1.66);score:(20.56±2.43)vs(5.14±1.71),F values were 5633.10,2224.37,2153.94,35.14,all P values<0.05].Through Pearson test,it was found that serum TGF-β1 level was negatively correlated with SLEDAI score(r=-0.802,P<0.05),serum IL-4,IFN-γwere positive correlated with SLEDAI score(r=0.825,0.813,both P values<0.05).The receiver operating characteristic curve analysis results show that the area under the curve for predicting the activity of systemic lupus erythematosus based on serum level of TGF-β1,IL-4 and IFN-γwere 0.872,0.892,0.905,the 95%CI were 0.785~0.933,0.809~0.948,0.825~0.957,with good predictive performance.ConclusionPatients with systemic lupus erythematosus.have significantly reduced serum TGF-β1 levels and significantly increased serum IL-4,IFN-γlevels,and the abnormal expression
