全内缝合技术治疗内侧半月板后根断裂的短期疗效

摘　　要：目的 研究全内缝合技术治疗内侧半月板后根断裂(medial meniscal posterior root tears, MMPRT)的短期疗效及功能评价。方法 回顾性队列分析应用全内缝合技术治疗25例MMPRT患者,评价指标包括术前及术后6月Lysholm、IKDC评分,术后3月复查MR,评估内侧半月板后根(medial meniscal posterior root, MMPR)愈合情况,术后6月复查McMurray试验、ADT试验、膝关节活动度等,记录患者术前、术后1月及6月VAS评分,记录手术时间、半月板缝合针数。结果 术中探查所有患者MMPRT诊断明确。除Ⅲ型及Ⅳ型其余患者缝合数为2针。术后3月及6月VAS评分均较术前改善(P<0.01)。术后6月IKDC、Lysholm评分均较术前显著提高(P<0.01)。术后6月1例McMurrary试验阳性,术后6月ADT试验均为阴性。术后3月3例MR提示MMPR缝合处2级信号,其余均为1级信号;术后6月1例膝关节活动轻度受限,患膝ROM 0~100°。结论 应用全内缝合技术修复MMPRT效果令人满意,术后愈合率高。Objective To investigate the short-term efficacy and functional evaluation of all-inside meniscal suture in the treatment of medial meniscus posterior root tears(MMPRT).Methods A retrospective cohort analysis was conducted to study the application of all-inside suture technology in the treatment of 25 MMPRT patients.Evaluation indicators included Lysholm and IKDC scores before surgery and 6 months after surgery.MR was reviewed 3 months after the surgery to evaluate MMPR healing.McMurray test,ADT test,and knee joint range of motion,were reviewed 6 months after the surgery.VAS scores were recorded before the surgery,1 month and 6 months after the surgery,and the operative time and number of meniscus suture needles were recorded.Results During the operation,MMPRT diagnosis was confirmed in all patients.Except for type III and IV patients,the number of sutures was 2;the VAS scores at 3 and 6 months after the surgery showed significant improvement compared to those before the surgery(P<0.01).After 6 months of the surgery,the IKDC and Lysholm scores were significantly improved compared to those before the surgery(P<0.01).One case tested positive for McMurrary test at 6 months after the surgery.After 6 months of the surgery,all ADT tests were negative.Three cases of MR showed grade 2 signal at the MMPR suture site at 3 months after the surgery,while the rest showed grade 1 signal.Six months after the surgery,one patient had mild limited knee joint mobility,with a knee ROM of 0-100°.Conclusion The application of all-inside suture technology for repairing MMPRT has a satisfactory effect and a high postoperative healing rate.
