虚实结合挂线术联合改良TROPIS术在高位复杂性肛瘘治疗中的应用效果观察

摘　　要：目的:观察虚实结合挂线术联合改良经肛括约肌间切开术(TROPIS)在高位复杂性肛瘘治疗中的应用效果。方法:将我院2022年7月至2023年12月收治的130例高位复杂性肛瘘患者随机分为对照组和观察组,每组65例。对照组患者采用虚实结合挂线术治疗,观察组在对照组基础上联合改良TROPIS治疗,比较2组患者的疗效、手术指标、术后疼痛情况以及术后并发症发生率。结果:观察组患者总有效率高于对照组(P<0.05);2组患者手术时间、术中出血量比较差异无统计学意义(P>0.05),观察组患者住院时间短于对照组(P>0.05);入组当天,2组患者NRS评分比较差异无统计学意义(P>0.05),术后7 d观察组患者疼痛评分低于对照组(P<0.05);观察组患者术后并发症发生率低于对照组(P<0.05)。结论:虚实结合挂线术联合改良TROPIS术治疗高位复杂性肛瘘患者,可缩短患者住院时间,有效缓解其术后疼痛,并能降低术后并发症发生率,效果显著。Objective To observe the effect of combination of dotted and solid thread-ligating therapy and modified transanal sphincter interposition incision(TROPIS)in the treatment of high complex anal fistulas.Methods A total of 130 patients with high complex anal fistula admitted to our hospital from July 2022 to December 2023 were randomly divided into control group and observation group,with 65 cases in each group.The control group was treated with the combination of dotted and solid thread-ligating therapy,and the observation group received the modified TROPIS on the basis of the control group.The efficacy,surgical indexes,postoperative pain and postoperative complication rate of both groups were com-pared.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).There was no significant difference in the operation time and intraoperative blood loss between the two groups(P>0.05),and the hospital stay in the observation group was shorter than that in the control group(P>0.05).On the day of enrollment,there was no significant difference in NRS score between the two groups(P>0.05),and the pain score of the observation group was lower than that of the control group 7 days after operation(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion The combina-tion of dotted and solid thread-ligating therapy combined with modified TROPIS in the treatment of patients with high complex anal fistulas can shorten the length of hospital stay,effectively relieve postoperative pain,and reduce the incidence of postoperative complications,with significant effect.
