自拟清热止痛生肌汤熏洗联合复方黄柏液涂剂在肛周脓肿患者术后的应用

摘　　要：目的:观察自拟清热止痛生肌汤熏洗联合复方黄柏液涂剂在肛周脓肿患者术后的应用效果。方法:选取2022年2月至2024年2月我院收治的100例肛周脓肿患者为研究对象,将其随机分为单药组和联合组,每组50例。单药组患者术后给予复方黄柏液涂剂治疗,联合组患者术后给予自拟清热止痛生肌汤熏洗联合复方黄柏液涂剂治疗,比较2组患者的疗效,记录患者症状改善时间、创面愈合时间及住院时间,比较不同时点患者疼痛评分、血管内皮生长因子(VEGF)、转化生长因子β(TGF-β)以及炎症因子水平。结果:术后14 d时,联合组患者总有效率显著高于单药组(P<0.05);联合组患者症状改善时间、创面愈合时间以及住院时间均显著短于单药组(P<0.05);术后1 d 2组患者疼痛评分比较差异无统计学意义(P>0.05),术后7 d、14 d时2组患者疼痛评分均显著降低,且联合组低于单药组(P<0.05);术后1 d 2组患者VEGF、TGF-β水平比较差异无统计学意义(P>0.05),术后14 d 2组患者VEGF、TGF-β水平均显著升高,且联合组高于单药组(P<0.05);术后1 d 2组患者炎症因子水平比较差异无统计学意义(P>0.05),术后14 d 2组患者炎症因子水平均显著降低,且联合组低于单药组(P<0.05)。结论:自拟清热止痛生肌汤熏洗联合复方黄柏液涂剂用于肛周脓肿患者术后能够提高患者的临床疗效,减轻患者疼痛,促进术后创面愈合。Objective To observe the application effect of fumigation and washing with self-designed Qingre Zhitong Shengji Decoction combined with Compound Huangbo Liquid Liniment in postoperative pa-tients with perianal abscess.Methods Total 100 patients with perianal abscess admitted to our hospital from February 2022 to February 2024 were selected as the study objects and randomly divided into single-drug group and combination group,with 50 cases in each group.Patients in the single-drug group were treated with Compound Huangbo Liquid Liniment after operation,and patients in the combination group received fumigation and washing with self-designed Qingre Zhitong Shengji Decoction combined with Com-pound Huangbo Liquid Liniment after operation.The efficacy of the two groups of patients were com-pared,and the symptom improvement time,wound healing time and hospital stay of the patients were re-corded.Pain scores,vascular endothelial growth factor(VEGF),transforming growth factor-β(TGF-β)and levels of inflammatory factors were compared at different time points.Results On the 14th day after operation,the total effective rate of the combination group was significantly higher than that of the single-drug group(P<0.05).Symptom improvement time,wound healing time and hospital stay in the combi-nation group were significantly shorter than those in the single-drug group(P<0.05).There was no sig-nificant difference in pain scores between the two groups on the 1st day after operation(P>0.05),and the pain scores of both groups were significantly decreased on the 7th and 14th day after operation,which were lower in the combination group than in the single-drug group(P<0.05).There was no significant difference in the levels of VEGF and TGF-βbetween the two groups on the 1st day after operation(P>0.05).The levels of VEGF and TGF-βin both groups on the 14th day after operation were significantly in-creased,which were higher in the combination group than in the single-drug group(P<0.05).There was no significant difference in the levels
