重症监护室危重症患者应用肠内营养安全护理路径对其营养水平的影响

摘  要:目的探讨肠内营养安全护理路径对重症监护室危重症患者营养水平的影响。方法回顾性分析2021年1月至2022年12月江南大学附属无锡五院和无锡市锡山人民医院收治的92例重症监护室危重症患者的资料,根据不同的干预方案分为对照组及观察组。对照组46例患者采用常规护理,观察组46例患者采用肠内营养安全护理路径干预。比较2组患者的营养水平、营养状况、预后、胃肠道功能失调发生率、营养支持后意外发生率和并发症发生率。结果护理7 d、14 d后,2组的白蛋白、总蛋白水平均高于同组护理前,欧洲营养风险筛查量表评分均低于护理前(P均<0.05),且观察组均优于同期对照组(P均<0.05)。观察组的机械通气时间、入住重症监护室时间和总住院时间均短于对照组(P均<0.05)。相较于对照组,观察组的胃肠道功能失调发生率、营养支持后意外发生率及并发症总发生率均较低(P均<0.05)。结论肠内营养安全护理路径可改善重症监护室危重症患者的营养水平与预后,降低其胃肠道功能失调、营养支持后意外和并发症的发生率。Objective To explore the effect of enteral nutrition safety nursing pathway on the nutritional level of critically ill patients in the intensive care unit.Methods A retrospective analysis was conducted on the data of 92 critically ill patients admitted to the intensive care units of Wuxi Fifth Hospital,Jiangnan University and Wuxi Xishan People's Hospital from January 2021 to December 2022.They were divided into a control group and an observation group according to different intervention plans.Forty-six patients in the control group received routine nursing,and forty-six patients in the observation group received enteral nutrition safety nursing pathway intervention.The nutritional level,nutritional status,prognosis,incidence of gastrointestinal dysfunction,incidence of accidents after nutritional support,and incidence of complications were compared between the two groups.Results After 7 and 14 days of nursing,the levels of albumin and total protein in both groups were higher than before nursing in the same group,and the scores of Nutritional Risk Screening 2002 in both groups were lower than before nursing(all P<0.05),and they were better in the observation group than in the control group at the same time(all P<0.05).The mechanical ventilation time,length of stay in the intensive care unit and total hospitalization time of the observation group were all shorter than those of the control group(all P<0.05).Compared with the control group,the incidence of gastrointestinal dysfunction,the incidence of accidents after nutritional support and the total incidence of complications in the observation group were lower(all P<0.05).Conclusion The enteral nutrition safety nursing pathway can improve the nutritional level and prognosis of critically ill patients in the intensive care unit,and reduce the incidence of gastrointestinal dysfunction,accidents and complication after nutritional support.
