七氟烷联合瑞芬太尼静吸复合全麻对腹腔镜子宫肌瘤剔除术麻醉效果的影响

摘　　要：目的探究七氟烷联合瑞芬太尼静吸复合全麻用于腹腔镜子宫肌瘤剔除术中对麻醉效果的影响。方法非随机选取2022年1月—2023年10月兖州区人民医院诊治的88例腹腔镜子宫肌瘤剔除术患者为研究对象,按照麻醉方法分为常规组(44例,丙泊酚+瑞芬太尼)与研究组(44例,七氟烷+瑞芬太尼),比较两组患者心血管应激反应情况、认知功能评分、术后不良反应发生情况。结果研究组患者在建立气腹0.5 h及术毕时的心率及平均动脉压均低于常规组,差异有统计学意义(P均<0.05);术后1 d,研究组患者的认知功能评分高于常规组,差异有统计学意义(P<0.05);研究组患者的不良反应发生率为6.82%(3/44),低于常规组的15.91%(7/44),差异有统计学意义(χ^(2)=4.062,P<0.05)。结论对腹腔镜子宫肌瘤剔除术患者施行七氟烷联合瑞芬太尼静吸复合全麻,能显著降低患者术中出现的心血管应激反应,降低术后不良反应发生率,且对患者的认知功能影响较小。Objective To explore the effect of sevoflurane combined with remifentanil intravenous inhalation com⁃bined general anesthesia on the anesthetic effect in laparoscopic myomectomy.Methods A total of eighty-eight pa⁃tients undergoing laparoscopic myomectomy in Yanzhou District People's Hospital from January 2022 to October 2023 were non-randomly selected as the research objects.According to the anesthesia method,they were divided into the routine group(forty-four cases,propofol+remifentanil)and the study group(forty-four cases,sevoflurane+remifent⁃anil).Cardiovascular stress response,cognitive function score and postoperative adverse reactions were compared be⁃tween the two groups.Results The heart rate and mean arterial pressure of the study group were lower than those of the conventional group at 0.5 h after pneumoperitoneum establishment and after operation,and the differences were statistically significant(all P<0.05).On the first day after operation,the cognitive function score of the study group was higher than that of the conventional group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group was 6.82%(3/44),which was lower than 15.91%(7/44)in the conventional group,the difference was statistically significant(χ^(2)=4.062,P<0.05).Conclusion Sevoflurane combined with remifent⁃anil inhalation combined with general anesthesia for patients undergoing laparoscopic myomectomy can significantly reduce the cardiovascular stress response and the incidence of postoperative adverse reactions,and has little impact on the cognitive function of patients.
