胆囊切除术后深部切口感染患者血清DAO、ET、D-Lac及免疫应激水平研究

摘　　要：目的探讨胆囊切除术后深部切口感染患者清二氨氧化酶(DAO)、内毒素(ET)和D-乳酸(D-Lac)变化对免疫应激水平的影响及临床意义。方法依据入排标准筛选符合要求的胆囊切除术后深部切口感染患者72例,酶联免疫吸附法(enzyme-linked immunosorbent assay,ELISA)测定血清DAO、ET和D-Lac和免疫球蛋白表达水平。流式细胞仪检测患者免疫细胞比例。此外检测并记录患者术前及术后1周内的体温、心率、WBC等常规指标水平以综合评估临床预后。结果术后1周患者的血清DAO、ET、D-Lac表达水平均显著升高,差异具有统计学意义(P<0.001)。免疫球蛋白表达水平和免疫细胞比例显著下调,差异具有统计学意义(P<0.001)。此外术后体温、心率和WBC等临床常规指标同样显著升高,差异具有统计学意义(P<0.001)。结论胆囊切除术后深部切口感染患者的血清DAO、ET和D-Lac水平显著升高并存在显著免疫抑制状态和生理应激反应。Objective To investigate the effects of changes of diamino oxidase(DAO),endotoxin(ET)and D-lactic acid(D-Lac)on immune stress level and its clinical significance in patients with deep infection after cholecystectomy.Methods According to the inclusion and exclusion criteria,72 patients with deep infection after cholecystectomy were selected.Serum DAO,ET,and D-Lac levels,as well as immunoglobulin expression levels,were measured using enzyme-linked immunosorbent assay(ELISA).The proportion of immune cells was detected using flow cytometry.Additionally,patients'body temperature,heart rate,and WBC levels were measured and recorded before surgery and within one week after surgery to comprehensively evaluate clinical prognosis.Results One week after surgery,the serum levels of DAO,ET,and D-Lac were significantly increased,with statistically significant differences(P<0.001).Immunoglobulin expression levels and the proportion of immune cells were significantly decreased,with statistically significant differences(P<0.001).Furthermore,postoperative clinical routine indicators such as body temperature,heart rate,and WBC were also significantly increased,with statistically significant differences(P<0.001).Conclusion Patients with deep infection after cholecystectomy showed significantly elevated serum DAO,ET,and D-Lac levels,along with significant immunosuppression and physiological stress responses.
