补肾活血方对肾虚血瘀型勃起功能障碍患者阴茎血流动力学的影响研究

摘　　要：目的观察补肾活血方对肾虚血瘀型勃起功能障碍(ED)阴茎血流动力学的影响,初步探讨补肾活血方治疗肾虚血瘀型ED的疗效和作用机制。方法回顾性分析2021年5月至2023年5月于潍坊市中医院泌尿男科门诊治疗的68例肾虚血瘀型ED患者的病历资料,根据治疗方式分成观察组(n=36)和对照组(n=32)。观察组予以补肾活血方联合他达拉非治疗,对照组予以他达拉非治疗,疗程4周。比较两组治疗前后国际勃起功能指数-5(IIEF-5)评分、勃起硬度评分(EHS)、勃起质量量表(EQS)评分、阴茎血流动力学参数、总有效率、不良反应发生率。结果两组治疗后IIEF-5、EHS、EQS评分及阴茎海绵体动脉收缩期峰值流速(PSV)和阻力指数(RI)均显著提高,舒张末期流速(EDV)显著降低,差异均具有统计学意义(P<0.05)。治疗后观察组IIEF-5、EHS、EQS评分及PSV、RI均显著高于对照组,EDV显著低于对照组(P<0.05)。观察组总有效率91.7%(33/36)高于对照组71.9%(23/32)(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论补肾活血方可以改善肾虚血瘀型ED患者的阴茎血流动力学,提高患者阴茎勃起功能。Objective To observe the effect of Bushen Huoxue Formula on the penile hemodynamics of erectile dysfunction(ED)with kidney deficiency and blood stasis type,and to preliminarily explore the efficacy and mechanism of Bushen Huoxue Formula in treating ED with kidney deficiency and blood stasis type.Methods The medical records of 68 erectile dysfunction patients with kidney deficiency and blood stasis type admitted to Department of Urology and Andrology,Weifang Traditional Chinese Medicine Hospital from May 2021 to May 2023 were retrospectively analyzed.According to the treatment,the patients were divided into observation group(n=36)and control group(n=32).The observation group received Bushen Huoxue Formula combined with tadalafil,and the control group was treated with tadalafil,with both course for 4 weeks.The international index of erectile function-5(IIEF-5)score,erection hardness score(EHS),erection quality scale(EQS)score,penile hemodynamic parameters,total effective rate,and incidence of adverse reactions before and after treatment were compared between the two groups.Results After treatment,the scores of IIEF-5,EHS,and EQS,the peak systolic velocity(PSV)and resistance index(RI)of the cavernous artery were all increased significantly(P<0.05),and end diastolic velocity(EDV)was significantly decreased,with statistically significant differences(P<0.05).After treatment,IIEF-5,EHS,and EQS,as well as PSV and RI in observation group were significantly higher than control group,and EDV was significantly lower than control group(P<0.05).The total effective rate was 91.7%in the observation group,which is higher than 71.9%in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Bushen Huoxue Formula can improve the penile hemodynamics of erectile dysfunction patients with kidney deficiency and blood stasis type,and improve the penile erectile function of patients.
