摘 要:目的 研究血清血管内皮生长因子(VEGF)、脂联素(ADPN)以及同型半胱氨酸(Hcy)水平与2型糖尿病(T2DM)微血管病变(MVP)的相关性.方法 回顾性分析茂名市人民医院在2022年3月-2023年10月期间收治的75例T2DM患者资料,将其按照是否并发MVP分作A组(并发MVP)43例及B组(未并发MVP)32例,另取同期健康体检者30例作为C组.对比三组各项临床资料及实验室指标水平.借助受试者工作特征(ROC)曲线评估空腹血糖(FBG)、VEGF、ADPN及Hcy水平预测T2DM并发MVP的效能.以Spearman相关性分析评估T2DM患者MVP与FBG、VEGF、ADPN及Hcy水平的相关性.结果 A组糖尿病病程长于B组(P<0.05).A组、B组、C组的FBG、VEGF、Hcy水平呈逐渐下降趋势,而ADPN水平呈逐渐升高趋势,且经单因素方差分析发现,各组间对比,差异有统计学意义(P<0.05).经ROC曲线分析证实,FBG、VEGF、ADPN及Hcy水平联合预测T2DM患者并发MVP的效能相较于单一指标明显更佳.经Spearman相关性分析证实,T2DM患者MVP与FBG、VEGF、Hcy水平均呈正相关关系,而与ADPN水平呈负相关关系(P均<0.05).结论 随着VEGF、Hcy水平的升高以及ADPN水平的降低,T2DM患者并发MVP的风险增加.Objective To investigate the correlation between serum vascular endothelial growth factor(VEGF),adiponectin(ADPN)and homocysteine(Hcy)levels and microangiopathy(MVP)in type 2 diabetes mellitus(T2DM).Methods The data of 75 T2DM patients admitted to Maoming People's Hospital from March 2022 to October 2023 were retrospectively analyzed.They were divided into group A(with concurrent MVP)43 cases and group B(without concurrent MVP)32 cases according to whether they had concurrent MVP.Another 30 healthy subjects in the same period were selected as group C.The clinical data and laboratory indicators of the three groups were compared.The efficacy of fasting blood glucose(FBG),VEGF,ADPN,and Hcy levels in predicting MVP in T2DM was evaluated using receiver operating characteristic(ROC)curves.Spearman correlation was used to analyze the correlation between the levels of MVP,FBG,VECF,ADPN and Hcy in T2DM patients.Results The course of diabetes in group A was longer than that in group B(P<0.05).The levels of FBG,VECF and Hcy in groups A,B and C showed a gradually decreasing trend,while the ADPN levels showed an increasing trend,and it was found by one-way analysis of variance that there was statistical significance among all groups(P<0.05).ROC curve analysis confirmed that the combined prediction of FBG,VEGF,ADPN and Hcy was significantly more effective in predicting MVP in T2DM patients than a single indicator.Spearman correlation analysis showed that MVP was positively correlated with FBG,VEGF and Hcy levels in T2DM patients,but negatively correlated with ADPN levels(all P<0.05).Conc lusion With the increase of VEGF and Hcy levels and the decrease of ADPN levels,the risk of co-occurring MVP in T2DM patients increases.
