摘  要:目的 评估肝脏实时剪切波弹性成像(SWE)联合超声半定量分析及血清谷丙转氨酶(ALT)在乙型肝炎(乙肝)患者肝脏纤维化程度评估中的临床应用价值.方法 选取2022年5月-2024年5月在我院就诊的乙肝患者作为研究对象,共计70例.所有患者均行肝脏SWE检查及超声半定量分析,并检测血清ALT水平.根据肝组织病理学检查结果,将患者分为纤维化程度不同的组别,分别为F0组(无纤维化)、F1组(轻度纤维化)、F2组(中度纤维化)、F3组(重度纤维化)和F4组(肝硬化).比较不同纤维化程度患者的SWE值、超声半定量分析结果和血清ALT水平,并进行相关性分析.利用受试者工作特征(ROC)曲线分析评估SWE值、超声半定量分析和血清ALT对乙肝患者肝脏纤维化程度的诊断价值.结果 乙肝患者肝脏纤维化程度与SWE值、超声半定量分析结果及血清ALT水平呈现显著相关性(P<0.05).ROC曲线分析显示,SWE值、超声半定量分析和血清ALT在评估乙肝患者肝脏纤维化程度中具有较高的诊断价值,且联合应用可以提高诊断的准确性.结论 肝脏SWE联合超声半定量分析及血清ALT检测可以作为评估乙肝患者肝脏纤维化程度的重要辅助手段,具有较高的临床应用价值.Objective To evaluate the clinical application value of real-time shear wave elastography(SWE)combined with ultrasound semi-quantitative analysis and serum alanine aminotransferase(ALT)in assessing liver fibrosis in patients with hepatitis B(HBV).Methods Seventy patients with hepatitis B,who were treated at our hospital from May 2022 to May 2024,were selected as the study subjects.All patients underwent liver SWE examination and ultrasound semi-quantitative analysis,and their serum ALT levels were measured.According to liver histopathological examination results,patients were divided into groups based on fibrosis stages:Fo(no fibrosis),F1(mild fibrosis),F2(moderate fibrosis),F3(severe fibrosis),and F4(cirrhosis).SWE values,ultrasound semi-quantitative analysis results,and serum ALT levels of patients with different fibrosis stages were compared,and correlation analysis was performed.The diagnostic value of SWE values,ultrasound semi-quantitative analysis,and serum ALT for liver fibrosis in hepatitis B patients was evaluated using receiver operating characteristic(ROC)curve analysis.Results There was a significant correlation between liver fibrosis stages in hepatitis B patients and SWE values,ultrasound semi-quantitative analysis results,and serum ALT levels(P<0.05).ROC curve analysis showed that SWE values,ultrasound semi-quantitative analysis,and serum ALT have high diagnostic value in assessing liver fibrosis in hepatitis B patients,and their combined application can improve diagnostic accuracy.Conclusion The combination of liver SWE,ultrasound semi-quantitative analysis,and serum ALT testing can serve as an important auxiliary method for evaluating liver fibrosis in patients with hepatitis B,demonstrating high clinical application value.
