土茯薏米猪苓汤治疗湿热型痛风性关节炎疗效观察

摘　　要：目的 评价土茯薏米猪苓汤治疗湿热型痛风性关节炎的临床疗效及安全性.方法 采用前瞻性随机对照试验设计,选择我院120例湿热型痛风性关节炎患者,随机等分为治疗组和对照组,每组60例.治疗组在常规西医治疗基础上加用土茯薏米猪苓汤;对照组仅接受常规西医治疗.治疗1个月后,比较两组中医证候积分、视觉模拟评分、血清尿酸、C反应蛋白水平变化,同时评估临床疗效和不良反应发生情况.结果 治疗组总有效率为91.67%,显著高于对照组的73.33%(P<0.05).治疗后,两组中医证候积分(除发热口渴外)、VAS评分及生化指标较治疗前均有不同程度降低(P<0.05),且治疗组优于对照组(P<0.05).治疗组未见明显不良反应.结论 土茯薏米猪苓汤能显著提高西药治疗湿热型痛风性关节炎的临床疗效,改善症状和炎症反应,且安全性良好,值得临床推广应用.Objective To evaluate the clinical efficacy and safety of Tufu Yimi Zhuling Decoction in the treatment of damp-heat type gouty arthritis.Methods A prospective,randomized,controlled study was conducted among 120 patients with damp-heat type gouty arthritis who were randomly divided into treatment and control groups.The treatment group received Tufu Yimi Zhuling Decoction plus conventional Western medicine,while the control group received conventional Western medicine only.After 1 month of treatment,changes in TCM syndrome scores,visual analog scale(VAS)scores,serum uric acid,C-reactive protein and inflammatory factors were compared between the two groups.The clinical efficacy and adverse reactions were evaluated.Results The total effective rate of the treatment group was 91.67%,which was significantly higher than 73.33%of the control group(P<0.05).Afer treatment,the TCM syndrome scores(except for fever and thirst),visual analogue scores and biochemical indexes of the two groups were lower than those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).There was no obvious adverse reaction in the treatment group.Conclusion Tufu Yimi Zhuling Decoction can significantly improve the clinical eficacy of Western medicine in the treatment of damp-heat type gouty arthritis by alleviating symptoms and inflammation,with good safety.It is worthwhile to promote the clinical application.
