脑脊液细胞学检查联合常规检验在临床诊断中的应用价值

摘　　要：目的分析脑脊液细胞学结合脑脊液常规提高临床的诊断价值。方法选取我院2021年1月至2023年12月收治的158例成人患者脑脊液标本,分别采用脑脊液细胞学检查和脑脊液常规检查,对比脑脊液细胞学检查、脑脊液常规检查单独检查与联合检查的结果。结果158份脑脊液标本中,脑脊液细胞学共计检出136份异常,异常率为86.08%,脑脊液常规检查共计检出113份异常,异常率为71.52%,联合检查共计检出150份异常,异常率为94.94%。脑脊液细胞学异常检出率、临床诊断符合性、特异性发现检出情况高于脑脊液常规检查(P<0.05),脑脊液细胞学结合脑脊液常规检查的异常检出率、临床诊断符合性、特异性发现检出情况高于单独检查(P<0.05)。脑脊液细胞学误判例数低于脑脊液常规检查(P<0.05),脑脊液细胞学结合脑脊液常规检查的误判例数低于单独检查(P<0.05)。结论与脑脊液常规检查相比,脑脊液细胞学检查具备更高的病情识别能力和诊断效能,采用脑脊液细胞学结合脑脊液常规检查则更能提高诊断效能。Objective To analyze the diagnostic value of cerebrospinal fluid cytology combined with cerebrospinal fluid routine.Methods Cerebrospinal fluid samples of 158 adult patients admitted to our hospital from January 2021 to December 2023 were selected,and cerebrospinal fluid cytology and routine cerebrospinal fluid examination were respectively used to compare the results of cerebrospinal fluid cytology and routine cerebrospinal fluid examination alone and combined.Results Among the 158 cerebrospinal fluid samples,136 abnormalities were detected by cerebrospinal fluid cytology,with an abnormal rate of 86.08%;113 abnormalities were detected by cerebrospinal fluid routine examination,with an abnormal rate of 71.52%;a total of 150 anomalies were detected during the joint inspection,with an abnormal rate of 94.94%.CSF cytological abnormality detection rate,clinical diagnosis conformity and specific detection rate were higher than that of routine CSF examination(P<0.05),and CSF cytology combined with routine CSF examination was higher than that of single examination(P<0.05).The number of cases of cerebrospinal fluid cytology misjudgment was lower than that of routine examination of cerebrospinal fluid(P<0.05),and the number of cases of cerebrospinal fluid cytology combined with routine examination of cerebrospinal fluid was lower than that of single examination(P<0.05).Conclusions Compared with the routine examination of cerebrospinal fluid,the cytology of cerebrospinal fluid has a higher ability of disease identification and diagnostic efficiency,and the combination of cerebrospinal fluid cytology and routine examination of cerebrospinal fluid can improve the diagnostic efficiency.
