摘　　要：目的对比输尿管软镜碎石(FURL)与微通道经皮肾镜碎石(mPCNL)治疗1.5~3 cm上尿路结石效果。方法选取2021年6月至2022年12月宜丰县人民医院收治的1.5~3 cm上尿路结石患者80例,其中采用FURL治疗的40例患者为研究组,采用mPCNL治疗的40例患者为对照组。对比两组手术时间、结石清除率、留置尿管、住院天数、住院费用及手术并发症发生情况。结果两组结石清除率及手术时间,差异无统计学意义(P>0.05)。研究组尿管留置时间、住院天数及手术并发症低于对照组,住院费用高于对照组,差异有统计学意义(P<0.05)。结论FURL与mPCNL治疗1.5~3 cm上尿路结石均为效果良好的手术方式,FURL与mPCNL相比手术时间短,创伤小,恢复快,有效缩短住院时间,减少术后并发症少,但在住院费用上FURL花费更多,术前需与患者及家属充分沟通。随着软镜技术逐步发展及负压吸引鞘的应用,FURL在处理1.5~3cm上尿路结石有可能逐步取代mPCNL。Objective To compare the effect of flexible ureteroscopic lithotripsy(FURL)and microchannel percutaneous nephroscopic lithotripsy(mPCNL)on 1.5-3 cm upper urinary stones.Methods The study selected 80 patients with 1.5-3 cm upper urinary tract stones from June 2021 to December 2022,of which 40 patients with FURL were the study group and 40 patients with mPCNL were the control group.Compare the surgical time,stone clearance rate,indwelling catheter,length of hospital stay,hospitalization costs,and incidence of surgical complications between two groups.Results There was no statistically significant difference in stone clearance rate and surgical time between the two groups(P>0.05).The study group had lower urinary catheter retention time,length of hospital stay,and surgical complications than the control group,and higher hospitalization costs than the control group,with statistically significant differences(P<0.05).Conclusions FURL and mPCNL treatment of 1.5-3 cm upper urinary calculi have good effect.Compared with mPCNL,FURL shorter operation time,less trauma,quick recovery,effectively shorten hospital stay,reduce postoperative complications,but in the hospital costs more,preoperative need to fully communicate with patients and their families.With the gradual development of soft lens technique and the application of negative pressure suction sheath,FURL may gradually replace mPCNL in the treatment of 1.5-3 cm upper urinary calculi.
