摘 要:目的随着过敏性疾病发生率的逐年升高,为提高预防效果,探讨分析血清总IgE(immunoglobulin E,IgE)水平、过敏原检测结果和过敏性疾病分布情况具有重要意义。方法病例组的50例来自首都医科大学附属北京世纪坛医院体检中心的体检者中的过敏性疾病患者,对照组为50例健康体检者。血清特异性免疫球蛋白E(specific immunoglobulin E,sIgE)使用免疫印迹法检测,血清总IgE浓度使用散射比浊法检测。计算19种过敏原的阳性率以及不同过敏原含量过敏性疾病的分布。结果病例组总IgE水平高于对照组,差异有统计学意义(P<0.05);病例组中过敏原阳性29例,其中单一过敏原阳性的13例,2种或2种以上(多重致敏)过敏原阳性的16例,阳性率为58.00%。过敏原阳性率占比排名前6位依次是艾蒿(34.00%)、葎草(16.00%)和屋尘(16.00%)、柳树/杨树/榆树(14.00%)、屋尘螨/粉尘螨(12.00%)和普通豚草(12.00%)。病例组中不同级别过敏性疾病的病种分布以过敏性鼻炎为主。结论在过敏性疾病的诊断中,血清总IgE可以作为一项辅助指标,而同时检测血清总IgE和特异性IgE(sIgE)能为诊断提供更为确凿的证据。Objective With the incidence of allergic diseases increasing yearly,in order to improve the preventive effect,it is of great significance to explore the dynamic trend of the total serum IgE(immunoglobulinE,IgE)level,the test results of allergens and the distribution of allergic disease.Methods 50 patients with allergic diseases from the Physical Examination Center of Beijing Shijitan Hospital of Capital Medical University were selected as the case group,while 50 healthy patients as the control group.The serum sIgE content was detected by immunoblotting method,and the total IgE content was detected by scattering turbidity method.The positive rate of 19 allergens and the distribution of allergic diseases with different allergens were calculated.Results The total IgE level in the case group was significantly higher than that in the control group(P<0.05);29 patients in the case group had positive allergens,including 13 patients with single positive allergens and 16 patients with positive allergens of 2 or more(multiple sensitization),with a positive rate of 58.00%.The positive rate of different types of allergens was shown in Table 3.The top six allergens with the highest positive rate were Artemisia(34.00%),Humulus lupulus(16.00%),House dust(16.00%),Populus/Populus alba/Ulmus(14.00%),Dermatophagoides farinae/Dermatophagoides pteronyssinus(12.00%)and Petasites hybridus(12.00%).Conclusion For the diagnosis of allergic diseases,the serum total IgE can be used as an auxiliary indicator,and the simultaneous detections of both the serum total IgE and specific IgE(sIgE)could provide stronger conclusive evidences for the diagnosis.
