摘　　要：目的硒是一种人体必备的微量元素,通常作为一种营养补充剂用于疾病的预防。然而,硒水平与自身免疫性甲状腺疾病的关系尚无定论。方法将采用双样本孟德尔随机化分析探讨硒水平与自身免疫性甲状腺疾病的关系。选择与血硒水平以及与指甲和血液(T&B)复合测量硒水平的相关的单核苷酸多态性(SNP)作为工具变量,选择来自Finngen与UK biobank人群的Graves病和桥本氏甲状腺炎作为结局,估计这些SNP与Graves病和桥本氏甲状腺炎的遗传关联。采用逆方差加权法作为主要分析方法。结果遗传预测的血硒水平与Graves病风险呈正相关(OR=1.21,95%CI 1.13~1.29,P<0.001)。遗传预测的T&B复合测量硒水平与Graves病风险呈正相关(OR=1.14,95%CI 1.01~1.27,P=0.028)。遗传预测的血硒水平与桥本氏甲状腺炎无关(OR=1.02,95%CI 0.98~1.07,P=0.370)。结果硒水平与Graves病风险增加有关,与桥本氏甲状腺炎无关。在普通人群中进行硒补充可能并不会降低甲状腺疾病的风险。Objective The association of selenium and autoimmune thyroid disease is remaining inconclusive and we aim to investigate this topic in the current study.Methods We used a two-sample mendelian randomization method to evaluate the impact of selenium on autoimmune thyroid disease.Single nucleotide polymorphisms(SNPs)strongly associated with blood selenium level and composite measure of toenail and blood(T&B)selenium level were selected as instrumental variables.Genetical associations of these SNPs with Graves’disease and Hashimoto’s thyroiditis were estimated.The inverse variance weighted method was applied as the primary analysis method.Results Genetically predicted blood selenium level was associated with increased risk of Graves’disease(OR=1.21,95 CI 1.13-1.29,P<0.001).It was nominal significant for the genetically predicted T&B selenium(OR=1.14,95%CI 1.01-1.27,P=0.028).The genetically predicted blood selenium level was not associated with Hashimoto’s thyroiditis(OR=1.02,95%CI 0.98-1.07,P=0.370).Conclusion Genetically predicted selenium is associated with increased risk of Graves’disease,while not reversely associated with autoimmune thyroid disease.
