摘　　要：目的:探讨二维斑点追踪超声心动图(2D-STE)参数评价妊娠期高血压心肌功能的临床价值,并分析其与血浆NT-proBNP水平的关系。方法:纳入35例子痫前期孕妇(PE组)、35例妊娠期高血压孕妇(GH组)及同期入院产检的35例健康孕妇(对照组)为研究对象。入院后24 h内,比较各组孕妇血浆中NT-proBNP水平;于药物控制血压前,比较各组孕妇常规心脏超声参数及2D-STE参数;分析PE组、GH组孕妇血浆NT-proBNP水平与2D-STE参数的相关性。结果:各组孕妇血浆NT-proBNP水平比较:PE组>GH组>对照组(P<0.05);各组孕妇LVEDV参数比较:PE组>GH组>对照组(P<0.05);LVESV、LVEF参数比较,PE组高于GH组和对照组(P<0.05);各组孕妇左心房整体纵向应变峰值、左心房收缩期应变率、左心房舒张早期应变率及左心室整体纵向应变峰值比较:PE组<GH组<对照组(P<0.05);左心室收缩期峰值心肌扭转角度比较:PE组>GH组>对照组(P<0.05)。PE组、GH组孕妇血浆NT-proBNP水平与左心房整体纵向应变峰值、左心房收缩期应变率、左心房舒张早期应变率及左心室整体纵向应变峰值均呈负相关关系(P<0.05),与左心室收缩期峰值心肌扭转角度呈正相关关系(P<0.05)。结论:2D-STE可用于评价妊娠期高血压疾病患者心肌功能损伤,与妊娠期高血压孕妇比较,子痫前期孕妇心肌功能损伤更为明显,且2D-STE参数与患者血浆NT-proBNP水平密切相关。Objective:To investigate the clinical value of two-dimensional speckle tracking echocardiography(2D-STE)parameters in evaluating myocardial function in hypertensive pregnancy,and to analyze the relationship between the parameters and plasma NT-proBNP level.Methods:35 cases of preeclampsia(PE group),35 cases of pregnancy-induced hypertension(GH group)and 35 healthy pregnant women(control group)during the same period were selected as the study objects.Within 24 h after admission,the plasma levels of NT-proBNP in all groups were compared.The parameters of routine cardiac ultrasound and 2D-STE were compared before blood pressure control.The correlation between plasma NT-proBNP level and 2D-STE parameters in PE group and GH group was analyzed.Results:The plasma NT-proBNP level of the three groups of pregnant women had statistical significance(P<0.05),and the PE group and GH group were higher than the control group(P<0.05),and the PE group was higher than the GH group(P<0.05).There were statistically significant differences in conventional cardiac ultrasound parameters among the three groups of pregnant women(P<0.05).LVEDV,LVESV and LVEF in PE group were higher than those in GH group and control group(P<0.05),and LVEDV in GH group was higher than that in control group(P<0.05).The overall longitudinal strain peak of left atrium,systolic strain rate of left atrium,early diastolic strain rate of left atrium and overall longitudinal strain peak of left ventricle in PE group and GH group were lower than those in control group(P<0.05),and the PE group was lower than that in GH group (P<0.05).The Angle of left ventricular peak systolic torsion in PE and GH groups was higher than that in control group(P<0.05),and PE group was higher than that in GH group(P<0.05).The levels of PE and GH plasma NT-proBNP in two groups of patients(PE group and GH group)were negatively correlated with the peak longitudinal strain of the left atrium,the systolic strain rate of the left atrium,the early diastolic strain rate of the left atrium and t
